RENAL CELL CARCINOMA : ANALYSIS OF 25 CASES

Twenty-five cases of renal cell carcinoma, diagnosed by surgical exploration and histopathologically at the Department of Urology of Ondokuz Mayıs University Faculty of Medicine between 1979 and 1989, have been reviewed retrospectively. Each patient was staged according to the TNM classification. Twenty of the patients underwent transabdominal nephrectomy and one underwent simple nephrectomy. The remaining four found to be inoperable upon exploration and only biopsies were performed for histopathological examination. Two patients underwent reexploration for solitary metastases and one for local recurrence. In advanced stages, including T3 , chemotherapy or hormonal therapy have been used also.
Evaluation of the results suggest that abdominal ultrasonography, performed for nonspesific symptoms, increases the incidence of the diagnosis of renal cell carcinoma at earlier stages, and survival depends on the stage of tumor at the time of diagnosis, rather than the type of therapy.




ÖZET

Ondokuz Mayıs Üniversitesi Tıp Fakültesi Üroloji Kliniğinde 1979-1989 yılları arasında, cerrahi girişimle histopatolojik olarak renal hücreli karsinom tanısı konan 25 olgu retrospektif olarak gözden geçirildi. Klinik, laboratuvar ve patolojik inceleme sonuçları değerlendirilerek TNM sınıflamasına göre evrelendirildi. Olguların 20'sine trans¬abdominal nefrektomi, birine basit nefrektomi uygulandı. Dördünde ise inoperable olduğundan sadece biyopsi almakla yetinildi. İki olgu metastazları ve bir olgu da lokal rekürrens nedeniyle reeksplore edildi. T3 dahil, ileri evre¬lerde kemoterapi veya hormonal tedavi de uygulandı.
Sonuçlar değerlendirildiğinde, nonspesifik bulgular için uygulanan abdominal ultrasonografinin renal hücreli karsinomu düşük evrelerde yakalama olasılığını yükselttiği ve yaşam süresinin uygulanan tedaviden ziyade, tümörün tanı konduğu andaki evresine bağlı olduğu sonucuna ulaşıldı.
