Sigara İçiminin Postoperatif Komplikasyonlara Etkisi

Öz

The Effects of Smoking on the Postoperative Complications.
\/ In this study, effects of smoking on the postoperative coughing, bronchospasm, sore throat and headache were investigated in 160 adult patients.
80 patients aged 18-75 (mean 48+18) were in the Nonsmoker group (Control) and 80 patients aged 22-72 (mean 39±13) were in the Smoker group. The two groups were si-milar in respect to age, ASA physical staus (I-II), duration of anaesthesia, use of opioid and anesthetic drugs and conditions of recovery.
All patients received atropine (0.01 mg/kg) + pethidine (1 mg/kg) for premedication, thiopental (6-8 mg/kg) + vecuronium (0.1 mg/kg) for induction and 02-N20-isoflurane (33-66-1%) for maintenance of anesthesia. Coughing and bronchospasm were evalua¬ted in one hour at recovery period, the patients were questioned for sore throat and headache after 24 hours. Data were compaired between the two groups. In the Non-smoker groups incidances of coughing were found 1.3%, 37.5%, bronchospasm 1.3%, 21.3% respectively. Differences between the two groups were highly signi¬ficant (p<0.001). Sore throat was seen 48.8% and 67.5% and the difference in this res¬pect was significant (p<0.01). There was no significant difference between the groups in the respect to headache.
As a result, smoking couses coughing, bronchospasm and sore throat in postoperative period and for this reason it should be given up when the operation was decided on.


%/ Bu çalışmada, sigara içiminin postoperatif öksürük, bronkospazm, baş ağrısı ve boğaz ağrısı üzerine etkileri 160 erişkin olguda araştırıldı.
Yaşlan 18-75 arasında (ort. 48+18) olan 80 olgu Sigara içmeyen grubu (kontrol), 22-72 arasında (ort. 39+13) olan 80 olgu da Sigara içen grubu oluşturdu, iki grup yaş, ASA fiziksel durum (I—II), anestezi süresi, narkotik ve anestetik ajan kullanımı ve ayılma koşulları bakımından benzer durumdaydı.
Olgulara premedikasyonda atropin (0.01 mg/kg) + petidin (1 mg/kg), indüksiyonda tiyopental (6-8 mg/kg) + veküronyum (0.1 mg/kg) ve anestezi idamesinde 02-N20-izofloran (%33-66-l) inhalasyonu uygulandı. Postoperatif dönemde bir saat süreyle derlenme odasında öksürük ve bronkospazm oluşması yönünden izlenen olgular 24 saat sonra da boğaz ağrısı ve baş ağrısı olup olmadığı yönünden sorgulandı. Elde edi¬len bulgular iki grup arasında karşılaştırıldı. Sigara içmeyen ve içen gruplarda sırasıyla öksürük %1.3 ve %37.5, bronkospazm % 1.3 ve %21.3 sıklıkta ve gruplar arası fark çok anlamlı bulundu (p<0.001). Aynı sırayla boğaz ağrısı %48.8 ve %67.5 sıklıklarda ve fark anlamlı (p<0.01) bulunurken baş ağrısı % 40.0 ve % 33.8 sıklıklarda, fark ise anlamsız (p>0.05) bulundu.
Sonuçta, sigara içiminin postoperatif derlenme sırasında önemli derecede öksürük ve bronkospazma, daha sonraki dönemde ise boğaz ağrısına yol açtığı, bu nedenle opera-syona gidecek hastalarda operasyon karan alındığı andan itibaren terkedilmesi gerek¬tiği kanısına varıldı.
