Bu çalışma, Türkiye’de ilaç, tıbbi cihaz ve biyoteknoloji gibi ürünlerden oluşan sağlık endüstrilerinin kurumsal, finansal ve mali yönlerden yeniden yapılandırılması ihtiyacını, savunma sanayiinde başarıyla uygulanan “karar-icra-finansman” mimarisi üzerinden ele almaktadır. Pandemi süreciyle beraber sağlık endüstrilerinin sadece birer sağlık ürün ve hizmet bileşeni değil aynı zamanda ekonomik güvenlik unsurları olduğu açık biçimde görülmüştür. Çalışma, Strateji ve Bütçe Başkanlığı (SBB), TİTCK, Kalkınma Planları ve Kalkınma Programları belgelerinden ve sektördeki STK raporlarından hareketle mevcut parçalı yapının, yüksek ithalat bağımlılığı ve düşük teknoloji derinliği nedeniyle sürdürülebilir kalkınma hedefleriyle uyumlu olmadığını ortaya koymaktadır. Bu kapsamda önerilen Sağlık Endüstrileri İcra Komitesi (SEİK), Sağlık Endüstrileri Başkanlığı (SEB) ve Sağlık Endüstrilerini Destekleme Fonu (SEDF) kurumsal yapılanmasını; strateji, uygulama ve finansman fonksiyonlarını aynı çatı altında birleştirerek karar-bütçe- sözleşme zincirini tek döngüde bütünleştirmeyi amaçlamaktadır. Model, AB’nin 2014/24/EU Kamu Alımları Direktifinde tanımlanan Toplam Sahip Olma Maliyeti (TCO) ve Değer Odaklı Tedarik (VBP) prensipleriyle uyumlu biçimde, bütçe verimliliğini artıran, öngörülebilir geri ödeme takvimleriyle finansman yükünü azaltan ve yerlileşme odaklı üretim kapasitesini destekleyen bir çerçeve sunmaktadır.
This study addresses the need for institutional, financial, and fiscal restructuring of Türkiye's healthcare industries, comprised of pharmaceuticals, medical devices, and biotechnology products, through the "decision-execution-financing" architecture successfully implemented in the defense industry. The pandemic has clearly demonstrated that healthcare industries are not only components of healthcare products and services but also elements of economic security. Drawing on documents from the Strategy and Budget Presidency (SBB), TİTCK, Development Plans and Development Programs, and reports from NGOs in the sector, the study demonstrates that the current fragmented structure is incompatible with sustainable development goals due to its high import dependency and low technological depth. In this context, the proposed institutional structure of the Health Industries Executive Committee (SEİK), Health Industries Presidency (SEB), and Health Industries Support Fund (SEDF) aims to integrate the decision-budget-contracting chain into a single cycle by uniting strategy, implementation, and financing functions under one roof. The model provides a framework that increases budget efficiency, reduces the financing burden with predictable repayment schedules, and supports localization-focused production capacity, in line with the Total Cost of Ownership (TCO) and Value-Based Procurement (VBP) principles defined in the EU's 2014/24/EU Public Procurement Directive.
