COMPARISON OF SURGICAL RESULTS OF RETROGRADE INTRAMEDULLARY NAILING AND FIXING WITH PLATE SCREW IN SUPRACONDYLAR FEMUR FRACTURES

ObjectiveSupracondylar femur fractures in adults are fracturesthat involve or do not involve the articular surface, andalthough there are different surgical fixation options,theyare a type of fracture that is not easy to treat. Sincethe soft tissue and periosteum viability could not bepreserved due to wide incisions in plate applicationsperformed in these fractures, and results such asnonunion were encountered, surgeons preferred theretrograde intramedullary nailing method.Material and MethodIn our study, two groups of patients who were fixedwith retrograde intramedullary nailing and plate werecompared. Twenty patients who were treated in ourclinic between 2005 and 2016 were included in thestudy, 11 of these patients were treated with platescrews and 9 with retrograde nails. The subjectivesatisfaction level of these patients was investigatedand compared in terms of postoperative VAS, HSS,Neer scores, development of arthrosis, quadricepsmuscle strength, thigh atrophy, and shortness.ResultsIn comparison, there was no statistically significantdifference between the two groups in terms ofsubjective satisfaction level, postoperative VAS, HSS,Neer scores, development of arthrosis, quadricepsmuscle strength, thigh atrophy, and shortness.ConclusionPlate and screw treatment and retrograde nailingmethods are reliable and good treatment methods insupracondylar femur fractures, both methods haveadvantages and disadvantages, but there is no majordifference between the two methods in terms of thepatient in the long run.

AmaçErişkinlerde görülen suprakondiler femur kırıkları eklemyüzeyini ilgilendiren veya ilgilendirmeyen kırıklarolup farklı cerrahi tespit seçenekleri bulunmasınarağmen, tedavisi kolay olmayan bir kırık çeşididir. Bukırıklarda yapılan plak uygulamalarında geniş insizyonlarsebebiyle yumuşak doku ve periost canlılığıkorunamayıp kaynamama gibi sonuçlarla karşılaşıldığındandolayı cerrahlar tarafından retrograd intramedullarçivileme metoduna bir yönelim meydanagelmiştir.Gereç ve YöntemÇalışmamızda retrograd intrameduller çivileme veplak ile tespit edilen iki grup hastada karşılaştırma yapıldı.2005-2016 tarihleri arasında kliniğimizde tedaviolan 20 hasta çalışmaya dahil edildi, bu hastaların 11’iplak ile 9’u ise retrograd çivi ile tedavi edildi. Bu hastalardasubjektif memnuniyet derecesi ameliyat sonrasıVAS, HSS, Neer skorları, artroz gelişimi, kuadrisepskası gücü, uyluk atrofisi ve kısalık açısından araştırıldıve karşılaştırıldı.BulgularYapılan karşılaştırmalarda her iki tedavi grubu arasındamemnuniyet derecesi postoperatif VAS, HSS,Neer skorları, artroz gelişimi, kuadriseps kası gücü,uyluk kas atrofisi ve kısalık açısından istatiksel olarakanlamlı bir fark saptanmadı.SonuçSuprakondiler femur kırıklarında plak-vida ile tedavive retrograd çivileme yöntemleri güvenilir tedavi yöntemleridirler,her iki yöntemin avantaj ve dezavantajlarımevcut olup uzun vadede değerlendirildiğinde heriki yönteminde suprakondiler femur kırıkları tedavisindebaşarılı olduğunu söyleyebiliriz.
