                                                                                                <abstract><p>AmaçKranyovertebral bileşkede (KVB) birçok farklı patolojigörülebilmektedir. En sık menengiom olmak üzere,schwannom, ependimom, hemanjioblastom bu bölgedeyerleşen kemik dışı kaynaklı benign lezyonlardır.Bu çalışmada, kliniğimize başvuran kranyovertebralbileşkede kemik dışı kaynaklı lezyonu olup, vertebralartere hakim olabilmek ve stabilizasyonun korunabilmesiiçin posterior yaklaşım yapılan ve enstrümankullanılmayan hastaların değerlendirilmesi amaçlanmıştır.Gereç ve YöntemSüleyman Demirel Üniversitesi Nöroşirürji kliniğine2020 yılında başvuran ve opere edilen 3 hastanın verilerideğerlendirildi. Hastaların 2’si erkek, 1’i kadın veyaş ortalamaları 47 idi (37-66). Posterior yaklaşımlahemilaminektomi sonrası intraoperatif nöromonitörkullanılarak mikrocerrahi ile intradural lezyonlar çıkarıldı.Bulgularİki hastada patolojik tanı schwannom, bir hastadaise menengiom olarak rapor edildi. Hastalarda postoperatifdönemde nörolojik defisit gelişmedi. Her 3vakada da başlangıç motor evoked potansiyel (MEP)değerlerine göre kapanış MEP değerlerinde düzelmegözlendi (%4,3; %6,7; %9,4 düzelme).SonuçBu bölge lezyonlarına anterior ve posteriordan farklışekillerde yaklaşımlar bildirilmiştir. Cerrahi tekniklerdeğerlendirilirken mümkün olan en geniş ve en güvenlitümör rezeksiyonu yapabilmek öncelikli amaçolmalıdır. Stabilitenin korunduğu dorsal yerleşimli lezyonlardaenstrümansız posterior yaklaşım tercih edilebilir.</p></abstract>

                            <p>ObjectiveVarious pathologies can be seen in craniovertebraljunction. Menengiomas, the most frequent, andschwannoma, ependimoma, hemangioblastoma arethe benign extra-osseous lesions localized in thisregion. In this study, we aimed to evaluate the patientsadmitted to our clinic with an extra-osseous lesion incraniovertebral junction, and operated via posteriorapproach to dominate vertebral artery and preservestabilisation, without an instrument.Material and MethodsThe data of 3 patients, admitted and operated inthe neurosurgery department of Süleyman DemirelUniversity, were evaluated. Two of the patientswere male, and one of them was female, and themean age was 47 (37-66). Microsurgical intradurallesion excision was done with posterior approachvia using intraoperative neuromonitoring afterhemilaminectomy.ResultsThe pathological diagnosis was schwannoma in 2patients and meningioma in 1 patient. No neurologicaldeficit was observed in the patients during postoperativeperiod. Improvement was observed in closure motorevoked potansiyel (MEP) values (4.3%; 6.7%; 9.4%improvement) compared with initial values in all patients.ConclusionVarious approaches from anterior and posterior werereported in this region lesions. Performing the mostpossible extensive and reliable tumor resection shouldbe the initial aim during the evaluation of surgicaltechniques. In the lesions localized dorsally posteriorapproach without instrumentation can be preferredwith the protection of the stability.</p></trans-abstract>
