<p>Objective: In abdominopelvic computed tomography (CT) with stone protocol, clinically important incidental findings (IF) can be encountered in organs other than stones. We aimed to investigate the frequency and importance of IF in patients who underwent CT for urolithiasis.Method: Between May and July 2024, 295 patients who came to the urology outpatient clinic with flank pain and suspected stones and underwent non-contrast abdominopelvic CT were retrospectively screened. Findings other than kidney and ureteral stones were evaluated as IF. IFs were grouped as urinary IF and extra-urinary IF. Age groups were divided into two groups as over 60 and 60 age and under. Results: Of 295 patients, 169 (57.3%) were male and 126 (42.7%) were female. The mean age was 47.86±12.8 years for males and 45.02±12.5 years for females. Urinary system stones were present in 69% of patients. Ifs were detected in 264 patients. 120 (40%) patients had 138 urinary IFs. Of these, 103 had one, 16 had two, and one had three. We detected 178 extra-urinary IFs in 144 (48.8%) patients. Of these, 114 had one, 27 had two, one had three, and four had two. The rate of extra-urinary IFs was higher in those under 60 years of age than in those over 60 years of age (p=0.02). No significant difference was observed in urinary IA rates by age. Furthermore, no significant difference was observed in incidental findings by gender. Among urinary findings, renal cysts were the most common. A renal mass, an adrenal mass, and a bladder mass, one case each, were significant in detecting malignancy. Among extra-urinary findings, gallstones and liver cysts were the most common. Conclusion: Abdominal CT scans performed for stone screening reveal IAs of the abdominopelvic organs, in addition to urolithiasis. Among these, malignant findings and conditions requiring prompt referral to relevant specialists are of particular importance. When focusing on urolithiasis, the evaluation of incidental findings should not be neglected.</p></trans-abstract>
