Obstetricians’ Knowledge and Practices On Mother-Infant Attachment: An Exploratory Study

Abstract

The quality of the mother-infant relationship in the immediate postpartum period can significantly influence the mother's mental health as well as the newborn's long-term well-being, development, and adaptation. Early skin-to-skin contact and the initiation of breastfeeding are crucial practices that promote mother-infant bonding and interaction. While obstetric nurses and midwives typically play a key role in facilitating these early interactions, obstetricians may be required to take on a more active role due to the limited number of nurses and midwives available. In such cases, obstetricians are expected to contribute to the initiation and continuation of the attachment process by adopting an appropriate approach toward the mother, the family, and the newborn. However, a review of the existing literature reveals a lack of research examining obstetricians' knowledge and practices regarding this issue. The present study aims to explore obstetricians’ level of knowledge about mother-infant attachment and the development of attachment in the postpartum period. To this end, semi-structured interviews were conducted with a total of 20 gynecologists (12 women and 8 men), aged between 26 and 64 years. Content analysis, a qualitative research technique, was used to analyze the data. The findings indicated that the participants were largely aware of the importance of practices such as skin-to-skin contact, early initiation of breastfeeding, and delayed cord clamping in supporting mother-infant attachment. The participants’ views suggest that the development of healthy mother-infant attachment is significantly influenced by the knowledge and competencies of healthcare professionals and the overall structure of the healthcare system. Furthermore, responses indicated a need for professional training on the subject of mother-infant attachment. It is believed that these findings may contribute to the development of policies by the World Health Organization and the Ministry of Health aimed at enhancing the quality of birth and postpartum care services, improving perinatal mental health, and supporting mothers' adaptation to their parenting roles.

KADIN DOĞUM UZMANLARININ ANNE-BEBEK BAĞLANMASI HAKKINDAKI BILGILERI VE UYGULAMALARI: KEŞIFSEL BIR ARAŞTIRMA

Abstract

Doğumdan sonraki ilk anlarda anne ile bebek ilişkisinin kalitesi annenin ruh sağlığını, yenidoğanın ise yaşamı boyunca devam eden iyilik halini, gelişimini ve uyumunu etkileyebilmektedir. Doğumdan hemen sonra anne ile bebek arasında ten tene temasın sağlanması ve emzirmenin başlatılması anne-bebek bağını ve etkileşimini desteklemektedir. Bu süreçte doğum hemşireleri ve ebelere önemli rol düşmekle birlikte, sayılarının yetersiz olması nedeniyle doğum sürecinde doğum uzmanları daha aktif rol üstlenebilmektedir. Bu noktada kadın doğum doktorlarının anne, aile ve bebeğe ihtiyaca uygun yaklaşımı ile bağlanma sürecini başlatma ve sürdürmede yardımcı olmaları beklenebilir. Ancak literatür taraması yapıldığında, kadın doğum uzmanlarının bu konudaki uygulamaları ve bilgilerinin ne durumda olduğuna dair bir çalışmaya rastlanmamıştır. Mevcut araştırmada, uzmanların anne-bebek bağlanması ve doğum sonrasındaki bağlanmanın gelişimi konusundaki bilgi düzeylerinin incelenmesi amaçlanmaktadır. Bu doğrultuda araştırmaya yaşları 26 ile 64 arasında değişen 12 kadın, 8 erkek olmak üzere toplam 20 kadın doğum uzmanı katılmış ve yarı-yapılandırılmış görüşmeler gerçekleştirilmiştir. Verilerin analizinde nitel analiz tekniklerinden biri olan içerik analizi kullanılmıştır. Bulgular uzmanların çoğunlukla ten tene temas, erken emzirme ve geç kord klemplemesi gibi uygulamaların bağlanmayı desteklemedeki rolünün farkında olduğunu göstermiştir. Katılımcıların görüşleri, anne-bebek bağlanmasının sağlıklı bir şekilde gelişebilmesi için sağlık sisteminin ve profesyonellerin bağlanma sürecini destekleyecek bilgi ve becerilere sahip olmasının belirleyici bir unsur olduğuna işaret etmektedir. Yine katılımcı yanıtlarına göre, uzmanların anne-bebek bağlanması konusunda eğitim ihtiyaçlarının bulunduğu sonucuna ulaşılmıştır. Elde edilen bulgularla, Dünya Sağlık Örgütünün ve Sağlık Bakanlığının doğum ve doğum sonrası bakıma yönelik hizmet kalitesini artırma, perinatal mental sağlığı iyileştirme, annelik ve ebeveynlik rolüne uyumu kolaylaştırma politikalarına katkı sağlanabileceği düşünülmektedir.
