Evaluation Of Analgesia Efficiency of Additional Local Anesthesia During The Circumcision Procedure

Abstract

Objective: Circumcision is the most common surgical procedure in the world and the pain during and after this procedure can affect the procedure comfort by leaving both the patient and his relatives and also the doctor in a difficult position. The aim of this study was to evaluate the efficacy of subcutaneous ring block local anesthesia on postoperative pain scores using VAS (visual pain score).

Material and Methods: Children over 5 years were divided into two groups for circumcision operation and subcutaneous ring block local anesthesia was applied to one group (Group 1). In the other group (Group 2) no local anesthesia was performed. All patients were compared using VAS (visual pain score). Results were statistically analyzed using Chi-square Test (p<0.05).


Conclusion: There were no significant difference of pain scores among the children who had subcutaneous ring block local anesthesia during the circumcision operation, the subcutaneous ring block was performed with local anesthesia; and the children who had not (p=0,675). However, while the patients with local anesthesia reported lower and moderate pain, patients who did not undergo local anesthesia expressed moderate and severe pain. It may be suggested to the physicians that, local anesthesia can also be offered during the circumcision as an alternative.

Sünnet İşlemi Sırasında İlave Lokal Anestezi Uygulamanın Postoperatif Analjezi Etkinliğinin Değerlendirilmesi

Abstract

Amaç: Sünnet işlemi Dünyada en çok uygulanan cerrahi işlem olup işlem sırasında ve sonrasında duyulan ağrı hem hastayı ve yakınlarını hem de hekimi zor durumda bırakıp işlemin konforunu etkileyebilmektedir. Bu çalışmanın amacı; sünnet operasyonu sırasında uyutulan çocuklara yapılan subcutan ring blok lokal anestezinin postoperatif ağrı skoruna olan etkisini VAS ( görsel ağrı skoru) kullanarak etkinliğini değerlendirmektir.

Gereç ve Yöntemler: 5 yaşından büyük çocuklara uyguladığımız sünnet operasyonunu iki gruba ayırarak bir gruba (Grup 1) çocuklar uyuduktan sonra lokal anestezik madde ile subcutan ring blok uygulandı. Diğer gruba (Grup 2) ise herhangi bir lokal anestezi işlemi uygulanmadı. Tüm hastalar VAS (görsel ağrı skoru) kullanılarak karşılaştırıldı. Bulgular istatistiksel olarak Ki-kare Testi kullanılarak analiz edildi (p<0.05)

Bulgular: Gruplar VAS sonuçları gözönüne alınarak değerlendirildiğinde subkutan ring blok yapılan hastalar (Grup 1) ile yapılmayan hastalar (Grup 2) arasında anlamlı farklılık bulunmadı (p=0.675) Grup 1 için ortalama VAS 3.09±0.26 iken , Grup 2 için bu değer 5.9±0.6 bulundu.

Sonuç: Sünnet operasyonu sırasında subcutan ring blok lokal anestezi uygulanan çocuklar ile; uygulanmayan çocuklar arasında ağrı skorları istatistiksel olarak fark göstermedi. Ancak lokal anestezi uygulanan hastalar daha çok düşük ve orta şiddetli ağrıyı bildirirken; lokal anestezi uygulanmayan hastalar orta ve şiddetli ağrı duyduklarını ifade ettiler. Çocukların daha az ağrı duyduğunu belirttiği, sünnet sırasında ilave olarak lokal anestezi uygulamak hekimlere alternatif olarak önerilebilir.
