Sepsisli Bir Bebekte Konfor Teorisine Dayalı Hemşirelik Bakımı: Olgu Sunumu

Abstract

Yenidoğan yoğun bakım üniteleri önemli sağlık sorunları olan yenidoğan bebeklerin tedavi edildiği teknolojik açıdan donanımlı birimlerdir. Bu teknolojik donanım bugün preterm doğan birçok bebeğin yaşama şansını arttırmıştır. Ancak hala enfeksiyon, preterm doğum, düşük doğum ağırlığı ve yoğun bakım ortamındaki olumsuz çevresel faktörler (ısı, ışık, ağrılı uyaranlar, vb.) prematüre bebeklerin gelişimlerini olumsuz etkilemekte yatış süresini uzatmakta dolayısıyla maliyeti arttırmaktadır. Literatürde çoğu çalışma hastanede yatışla paralel olarak stres ve travmanın prematüre bebekler için uzun vadede olumsuz psikososyal etkilerinin olabileceği belirtilmiştir. Olgu 35+6 GH preterm bebek sepsis tanısıyla takip edilmektedir. Üçüncü düzey yenidoğan yoğun bakım ünitesinde ilk yatış ve sonrasında post-op takip edilen preterm bebek A’nın hemşirelik bakım süreci 50 gündür devam etmektedir. İlk değerlendirmede ağrı skoru yüksek, konfor düzeyi düşük belirlenen bebeğin bakımdan primer sorumlu olan hemşirelerin bebeğe verdiği hemşirelik bakımı kritik rol oynamaktadır. Hemşirelik kuramlarına dayalı uygulanacak bir bakım planı bebeğin konforunu arttırarak refahına kavuşması için yol gösterici olmaktadır. Prematüre bebeği bütüncül bakış açısıyla değerlendirerek, bebeğe özgü hastalık girdilerini belirlemek ve bakımı sistematize etmek arasındaki ilişki oldukça önemlidir. Kolcaba’nın Konfor kuramını kullanarak oluşturduğumuz bakım planının, sepsisli preterm bir bebeğin sağlık çıktılarına etkisini incelediğimiz bu çalışmada kurama dayalı oluşturduğumuz girişimler bebeğin konfor düzeyini yükselterek ağrı puanını düşürmüştür.

Nursing Care Based on Comfort Theory In An Infant with Sepsis: Case Report

Abstract

Neonatal intensive care units are technologically equipped units where infants with important health problems are treated. Being technologically equipped has increased the chances of survival of many preterm infatns today. However, infection, preterm birth, low birth weight, and adverse environmental factors (heat, light, painful stimuli...) in the intensive care environment affect the development of premature infants negatively, prolonging the length of hospital stay, thus increasing the cost. Many studies in the literature have indicated that stress and trauma may have long-term negative psychosocial effects for premature infants in parallel with hospitalization. The case is being followed up with the diagnosis of 35+6 GH preterm infant sepsis. The nursing care process of preterm infant A, who was hospitalized in the third level neonatal intensive care unit and followed up post-op, continues for 50 days. Nursing care given to the infant by the nurses who are primarily responsible for the care of the infant, whose pain score is high and the comfort level is low in the first evaluation, plays a critical role. A care plan to be applied based on nursing theories will increase the comfort of the infant and guide him to achieve well-being. The relationship between assessing the premature infant from a holistic perspective, identifying baby-specific disease inputs and systematizing care is very important.
