Comparison of VMAT and IMRT Treatment Planning Techniques for Prostate Cancer

Abstract

Çalışmamızın amacı; Prostat tedavisinde VMAT ve IMRT planlarını karşılaştırmaktır. Total doz 37 fraksiyonda 74 Gy olacak şekilde VMAT ve IMRT ile 20 prostat planı oluşturuldu. Karşılaştırmada düşük doz bölgesi, monitör unit(MU), OARs'lerin referans dozları ve tümör hacim dozları kullanıldı. Ayrıca doz dağılımlarının uygunluğu yeni ve evrensel CI algoritması ile değerlendirildi.
VMAT ve IMRT planlarında PTV için %V95, V100% ve ortalama dozlar birbirine çok yakındı ve sonuçlar İstatistiksel olarak anlamlı değildi (p>0,05). VMAT tekniği ile neredeyse tüm referans dozlar için mesanede daha düşük dozlar elde edildi. Bununla birlikte, IMRT tekniği rektum için VMAT'tan daha düşük dozlara sahipti. Tüm OAR referans dozlarında IMRT ve VMAT arasında klinik olarak üstünlük yoktu. Ancak doz dağılımlarının konformitesi arasındaki fark klinik ve istatistiksel olarak anlamlı bulundu (VMAT'ta %95 IDS için konformite:%82 ve IMRT'de %95 için konformite %72 ) (p<0,001). Ayrıca VMAT tekniği MU karşılaştırmasında IMRT tekniğine göre %35 azalma ile klinik ve istatistiksel olarak anlamlı bulunmuştur (p<0,001). Son olarak ta IMRT'ye göre VMAT'ın düşük doz bölgesinde daha iyi sonuçlar verdiği anlaşıldı. VMAT tekniğinin IMRT tekniğine üstünlüğü ideal, özgün ve evrensel Conformity Index algoritması sayesinde açıkça ortaya konmuştur. Ayrıca, en iyi planın seçimi için DVH'in yanı sıra her koşulda doğru sonuç veren ideal Konformite İndeks algoritması da kullanılmış olmalıdır.
