Oral Antikanser İlaç Kulanan Kanser Hastalarında Telefon ile Yapılan İzlemin İlaç Uyumuna Etkisi: Sistematik Derleme

Öz

Amaç: Kanser görülme sıklığının artması ile beraber, tedavi yöntemleri de gelişmiştir. Bu
gelişmelerden biride kemoterapik ilaçların oral formlarının artmasıdır. Bu çalışmada, oral
antikanser ilaç kullanan kanser hastalarında telefon ile yapılan izlemin ilaç uyumuna etkisinin
sistematik olarak incelenmesi amaçlanmıştır.
Gereç ve Yöntem: Oral antikanser ilaç kullanan kanser hastalarında telefon ile yapılan izlemin
ilaç uyumuna etkisini ortaya koyabilmek için, 2007 ile 2017 Ocak tarihleri arasında yayınlanmış
4858 çalışma taranmıştır. Araştırma kriterlerine uyan yedi çalışma incelenmiştir. Seçilen
araştırmalar, türlerine göre Joanna Briggs Enstitüsü (JBI) eleştirel değerlendirme listelerine
göre değerlendirilmiştir.
Bulgular: Araştırmada incelenen yedi çalışmanın (prospektif çalışma: 1, tanımlayıcı çalışma:
2, Kohort çalışma: 1, randomize kontrollü çalışma: 1, yarı deneysel çalışma: 2) kalitesi orta
derecede bulunmuştur. İncelenen makalelerde hastaların ilaç uyumunu etkileyici faktörlerin
ve uyumun ölçülmesine yönelik yöntemlerin belirsiz ve ortak olmadığı saptanmıştır. Ayrıca
çalışmalarda ilaç uyumu oranları, hasta örneğine, ilaç türüne, takip periyoduna, değerlendirme
ölçüsüne ve uyum hesaplanmasına bağlı olarak çeşitlilik gösterdiği belirlenmiştir.
Sonuç: Telefon ile izlemin oral kemoterapi kullanan hastalarda ilaç uyumunu artırma konusunda
kanıtlar sınırlı kalmaktadır. İleride yapılacak kaliteli çalışmalara ihtiyaç vardır. Sağlık ekibi
hasta eğitimi, hastaların tedavi planlarını gözden geçirmek ve belgelemek, oral kemoterapi ilaç
kullanımı uyumu için rutin hasta izlemi konularında standart prosedürler geliştirmelidir.
