Clınıcal Solutıon for Non-Maıntaıned Bılateral Balanced Artıculatıon of Implant Supported Overdenture Prostheses: Two Case Reports

Öz

It is aimed to reorganize aesthetics, phonation and function in prosthetic treatments. Pre-treatment patient demands and post-treatment complaints may affect their priority order.
In the first case report; The treatment of a 55-year-old female case of complete edentulism with implant-supported overdenture prosthesis with lower and upper (upper 4, lower 3 implants) was described. The treatment of the patient, whose treatment was completed with a protocol different from the conventional steps, was completed using a pantographic recorder and a fully adjustable articulator programmed with the angular values of the dynamic data of the joint.
In the second case report; The treatment of a 65-year-old female patient who wanted the replacement of her existing prostheses with a total prosthesis in the edentulous upper jaw and an implant-supported overdenture prosthesis in the lower jaw was described. The treatment was completed with a protocol similar to the first case report. The pink and white aesthetics obtained in the final prostheses of both cases are satisfactory. However, despite the final aesthetics of the prostheses, early contacts in articulation led to dissatisfaction.
In both cases, after the delivery of the dentures, the patients complained that their dentures were displaced in certain tooth contacts. In order to provide the necessary dynamic stability, dynamic facet repairs were performed with direct intraoral detection in both case reports. Dynamic stability was achieved at the end of 6 weeks in case 1 and at the time of delivery in case 2.

İmplant Destekli Overdenture Protezlerde Sağlanamayan Bilateral Balanslı Artikülasyon ve Klinik Çözümü: İki Olgu Sunumu

Öz

Protetik planlama ve tedavilerde estetik, fonasyon ve fonksiyonun yeniden düzenlenmesi hedeflenmektedir. Tedavi öncesi hasta talepleri ve tedavi sonrası şikayetler bunların öncelik sıralamasını etkileyebilmektedir.
Birinci olgu sunumunda; 55 yaşında kadın tam dişsizlik olgusunun, alt üst (üst 4, alt 3 adet) implant destekli overdenture protezler ile olan tedavisi anlatıldı. Konvansiyonel adımlardan farklı bir protokolle tedavisi tamamlanan hastanın tedavisi pantografik kayıt cihazı ile eklemine ait dinamik verilerin açısal değerleri ile programlı tam ayarlanabilir artikülatör kullanılarak tamamlandı.
İkinci olgu sunumunda; Mevcut protezlerinin yenilenmesini isteyen 65 yaşında kadın hastanın, tam dişsiz üst çenenin total protez ile, alt çenenin implant destekli overdenture protez ile olan tedavisi anlatıldı. Tedavi ilk olgu sunumuna benzer bir protokolle tamamlandı. Her iki vakanın final protezlerinde elde edilen pembe ve beyaz estetik memnuniyet vericidir. Ancak protezlerin final estetiğine rağmen artikülasyondaki erken temaslar memnuniyetsizliğe yol açmıştır.
Her iki olguda da protezlerin teslimi sonrası hastaların protezlerinin belirli diş temaslarında yerinden oynadığı yönünde şikayetleri oldu. Gerekli dinamik stabilitenin sağlanabilmesi için her iki olgu sunumunda da direkt ağız içi tespitle dinamik faset onarımları yapıldı. 1. olguda dinamik stabilite 6 haftanın sonunda, 2. olguda ise teslim anında sağlanabildi.
