Kafa Travmasına Bağlı Benign Paroksismal Pozisyonel Vertigo ve Klinik Özellikleri

Öz

Amaç: Çalışmanın amacı kafa travması sonrası görülen Benign Paroksismal Pozisyonel Vertigo (BPPV)’nun tutulan kanal özelliklerini, tedavi yaklaşımını ve etkinliğini araştırmaktır.
Gereç ve Yöntemler: Çalışmaya 80 BPPV hastası arasından trafik kazası, düşme, başa sert cisimle darbe alma vb. nedenlerle kafa travması geçirmiş 15 birey (%18,75) alınmıştır. Çalışmada Dix-Hallpike ve Roll testleri sonrası BPPV tanısı alan 6 kadın (39,6618,04 yıl), 9 erkek (4510,5 yıl) dahil edilmiştir. Katılımcılara BPPV’de etkilenen taraf ve kanal kanal/lar belirlendikten sonra bunlara uygun kanalit repozisyon manevraları uygulanmıştır.
Bulgular: Çalışma sonucunda sağ kulak etkilenimi %47,6, sol kulak etkilenimi %52,4 olarak elde edilmiştir. Katılımcıların %53,3’ünde posterior semisirküler kanal, %20’sinde horizontal semisirküler kanal, %6,7’sinde anterior semisirküler kanal etkilenimi görülürken, %20’sinde Horizontal ve posterior kanal etkilenimi birlikte görülmüştür. BPPV sonrası uygulanan manevra sayısı ile yaş arasında (r=0,605, p=0,017) ve manevra sayısı ile tutulan kanal sayısı arasında pozitif yönde orta derece korelasyon bulunmuştur (r=0,580, p=0,023).
Sonuç: Kafa travması sonrası görülen Benign paroksismal pozisyonel vertigo kanalların etkilenme oranı, tedavi yaklaşımları ve tedavi etkinliği ile idiopatik BPPV’den farklılıklar göstermektedir. Özellikle çoklu kanal tutulumuna bağlı tedavi süresi uzayabilmektedir.
