Uterusun Adenomatoid Tümörü: Nadir Görülen ve Leiomyom ile Karışabilen Olgunun Sunumu

Abstract

Adenomatoid tumors are rarely seen in the uterus and are often clinically, radiologically and macroscopically misinterpreted as leiomyomas. Histologically distinction from epithelial tumors is needed in hematoxlin and eosin sections. The immunohisto-chemical and ultrastructural studies suggest that these neoplasms have a mesothelial origin. İn this study vve report a 48 year-old patient vvith an adenomatoid tumor in the uterin corpus who vvas operated vvith the preoperative diagnosis of leiomyoma. Immu-nohistochemically, the neoplastic cells vvere stained strongly positive vvith pancytokeratin and the mesothelial antigen calretinin and HBME-1. EMA, desmin, actin, MOC-31, Ber-EP4, CD 15 and CD 31 vvere consistently negative. The immunohistochemicai results support the diagnosis of adenomatoid tumor and thus, the mesothelial origin.

Abstract

Adenomatoid tümörler uterusun nadir görülen benign tümörleridir. Klinik, radyolojik ve makroskobik olarak leiomyom ile karışırlar. Histopatolojik olarak hemotoksiien-eosin kesitlerde epitelyal tümörlerden ayrımı gerekebilir. İmmünhistokimyasal ve ultrast-rüktürel çalışmalar bu tümörün mezotelyal kökenli olduğunu göstermektedir. Bu çalışmada leiomyom ön tanısı ile öpere edilmiş 48 yaşında bayan hastada uterus korpusunda saptanan adenomatoid tümör olgusu sunulmuştur, immünhistokimyasal olarak tümör hücreleri pansitokeratin ve mezotelyal antijen kalretinin ve HBME-1 ile kuvvetli pozitif boyandı. EM A, desmin, aktin, MOC-31, Ber-EP4, CD 15 ve CD 31 ile boyanma görülmedi. Bu sonuçlar olgunun adenomatoid tümör tanısını ve mezotelyal kökeni desteklemektedir.
