Gingivitisli Hastalarda Cerrahi Olmayan Periodontal Tedavinin Tükürük PTX-3 Seviyesine Etkisinin Değerlendirilmesi

Abstract

Amaç: Akut faz proteinleri ailesinin bir üyesi olan pentraksin-3 (PTX-3), çeşitli sitokinlerin etkisi ile endotel hücreleri, fibroblastlar, makrofajlar ve nötrofiller gibi enflamasyon bölgesinde bulunan hücrelerden salınmaktadır. Sağlıklı bireylerde düşük olduğu bilinen plazma PTX-3 konsantrasyonları kronik enflamatuvar hastalıklarda artış göstermektedir. Bu çalışmada gingivitisli hastalarda cerrahi olmayan periodontal tedavinin (COPT) tükürük PTX-3 seviyeleri üzerine etkisinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Çalışmaya 13 gingivitisli hasta (gingivitis grubu) ve 13 periodontal sağlıklı birey (kontrol grubu) olmak üzere toplam 26 birey dahil edildi. Çalışmanın başlangıcında tüm bireylerden plak indeks, gingival indeks, sondalamada kanama ve sondalama derinliğinden oluşan periodontal klinik parametreler ölçülerek tükürük örnekleri toplandı. Gingivitis grubuna COPT uygulandı. COPT sonrası 3. ayda gingivitis grubunda klinik ölçümler ve tükürük örneklerinin toplanması tekrarlandı. Tükürük PTX-3 seviyeleri ELISA yöntemiyle saptandı. Bulgular: Gingivitis grubunda tüm periodontal klinik parametrelerin başlangıçta kontrol grubuna kıyasla daha yüksek olduğu (p<0,001) ve COPT sonrası istatistiksel anlamlı azalma gösterdiği saptandı (p<0,01). Başlangıçta gingivitis grubunda tükürük PTX-3 seviyesi kontrol grubuna kıyasla anlamlı derecede yüksek bulundu (p<0,05). Gingivitis grubunda COPT sonrası tükürük PTX-3 seviyesi tedavi öncesine kıyasla azalmakla birlikte aralarındaki fark anlamlı bulunmadı (p>0,05). Sonuç: PTX-3’ün kronik enflamatuvar bir hastalık olan gingivitisin patogenezinde rolü olabileceği gösterilmiş olmakla birlikte COPT’nin tükürük PTX-3 seviyelerine etkisinin açıklığa kavuşturulabilmesi için daha fazla çalışmaya ihtiyaç olduğu sonucuna varılmıştır.

Effect of Non-Surgical Periodontal Treatment On Salivary PTX-3 Level In Gingivitis Patients

Abstract

Objectives: Pentraxin-3 (PTX-3), a member of the acute phase protein family, is released from cells such as endothelial cells, fibroblasts, macrophages, and neutrophils induced by cytokines in the inflammation region. Plasma PTX-3 concentrations, known to be low in healthy individuals, increase in chronic inflammatory diseases. This study aimed to evaluate the effect of non-surgical periodontal therapy (NSPT) on salivary PTX-3 levels in patients with gingivitis. Material and Methods: A total of 26 subjects, including 13 gingivitis patients (gingivitis group), and 13 periodontally healthy individuals (control group), were included in the study. At the beginning of the study, saliva samples were collected, and periodontal clinical parameters including plaque index, gingival index, bleeding on probing, and probing depth were measured from all individuals. NSPT was applied to the gingivitis group. Clinical measurements and sample collection were repeated in the gingivitis group 3 months after NSPT. PTX-3 levels in saliva were determined by ELISA method. Result: All periodontal clinical parameters were found to be higher in the gingivitis group compared to the control group at baseline (p<0,001), and showed a statistically significant decrease after NSPT (p<0,01). Salivary PTX-3 level was significantly higher in the gingivitis group than the control group at baseline (p<0,05). Although salivary PTX-3 level decreased after NSPT in the gingivitis group, there was no significant difference between baseline and after 3 months after NSPT (p>0,05). Conclusions: It was demonstrated that PTX-3 may play a role in the pathogenesis of gingivitis. However, it was concluded that further studies are needed to clarify the effect of NSPT on salivary PTX-3 level.
