MAKSİLLER SİNUSE KOMŞU BİR RADİKULER KİSTİN ENUKLEASYONU VE SPONTAN İYİLEŞMENİN ARDINDAN BÖLGEYE YÖNLENDİRİLMİŞ KEMİK REJENERASYONU İLE EŞ ZAMANLI DENTALİMPLANT UYGULANMASI

Abstract

Periapikal kist veya apikal periodontal kist olarak da bilinen radiküler kist (RC) tüm odontojenik kistlerin %50’sinden fazlasını oluşturan, çene kemiklerinin en sık görülen inflamatuar odontojenik kistik lezyonudur. Çoğunlukla asemptomatik olan bu kistler belirli boyutlara ulaştıklarında bölgede ciddi kemik defektlerine sebep olabilir. Günümüzde özellikle dişsiz sonlanan vakalarda, diş eksikliklerinin telafisinde, altın standart halini alan dental implant uygulamalarında başarı için en önemli faktörlerden biri bölgede yeterli hacim ve kalitede kemik dokusu bulunmasıdır. Bu nedenle büyük boyutlu çene kistlerin enükleasyonunun ardından bölgede oluşan kemik defektinin onarımı için bölgenin greftlenmesi ya da doğal iyileşme sürecine bırakılması son yıllarda daha sık tartışılan bir konu halini almıştır. Bu olgu sunumunda maksilla posterior bölgede gelişen ve maksiller sinüs ile komşuluğu bulunan büyük boyutlardaki radiküler kistin enükleasyonu ardından bölgenin spontan iyileşme sürecine bırakılması, bölgede yeni oluşan kemik ve skar dokularının CBCT ile görüntülenmesi ve daha sonra bölgenin protetik rehabilitasyonu amacıyla yönlendirilmiş kemik rejenerasyonu ile eş zamanlı dental implant uygulamasının 1 yıllık takibi sunulmuştur.
