Siyanür zehirlenmesi ile meydana gelen ölümlerde toksikolojik analizin önemi: Olgu sunumu

Abstract

Siyanür eski çağlardan beri bilinen, küçük dozlarda bile dakikalar içinde ölüme neden olabilen bir zehirdir. Siyanür zehirlenmesine bağlı ölümler, spesifik bulguların olmaması sebebiyle ölüm nedeni tespiti zor olgular arasında yer alır. Çalışmada; sokakta, yerde yatar pozisyonda kasılı halde bulunan ve tanıkların ifadelerine göre; ölen kişinin maddi sıkıntı içinde olduğu, eski eşinden boşanamadığı için resmi bir sözleşme olmadan başka bir kadınla birlikte aynı evde yaşadığı, birlikte yaşadığı kadın arkadaşı ile sık sık tartıştığı ve ilaç içerek kendini zehirleyeceğini çevresine ifade ettiği belirtilmektedir. Genel durumunun kötüleşmesi ve şuurunun kapanması üzerine ambulansla hastaneye getirilen ve hastanede ölen 52 yaşındaki erkek olgu otopsi ve toksikolojik analiz bulguları ile sunulmaktadır. Çalışmanın sonuçlarına göre; siyanür zehirlenmelerinin aydınlatılmasında otopsinin ve histopatolojik bulguların tek başına yeterli olamayabileceği, toksikolojik analizlerin tanısal değerinin ölüm nedeninin saptanmasında büyük önem taşıdığı ortaya konulmuştur. Bu çalışmada; siyanür zehirlenmesi nedeniyle gerçekleşen ölüm olgularında toksikolojik analizin ölüm nedeninin saptanmasındaki yeri ve öneminin vurgulanması amaçlanmıştır.

The importance of the toxicological analysis of death cases caused by cyanide poisoning: A case report

Abstract

For centuries, cyanide has been known as a powerful poison, which is quick-acting and can cause death within minutes even at small doses. As there are no specific findings in deaths associated with cyanide poisoning, they are one of the most difficult cases to determine in autopsy. The aim of this study was to emphasize the relevance and importance of toxicology analysis in the determination of cause of death due to cyanide poisoning. In the witness statements it was reported that the deceased had a substance abuse problem, he was divorced and living with another woman, but not officially married. There were frequent arguments between him and the woman with whom he was living and he stated that he had taken the drug to poison himself. The findings of the autopsy and toxicology analysis of a 52-year old male, who was found lying in the street stating that he had ingested cyanide, are presented. He was in poor health condition and was on the verge of losing consciousness. Despite all the necessary interventions, he died while being taken to the hospital. According to the results of this study, autopsy and histopathological findings may not be sufficient in the clarification of the cases related to cyanide poisoning and the most objective finding was seen to be the toxicology analysis.
