Evaluation of forensic psychiatry cases referred to psychiatry department of a university hospital

Abstract

OBJECTIVE: The aim of this study is to investigate in a retrospective manner the socio-demographic features, psychiatric diagnoses of the forensic cases dispatched to our psychiatry unit and the reason of why such cases were subject to forensic psychiatry evaluation. METHODS: The medical reports of 456 patients that were sent to the psychiatry unit of the university by forensic institutions between the dates of January 2012-November 2015 were retrospectively analyzed. Afore-mentioned reports belong to the age group of 18 and older. These reports, solicited bu the penal law and civil law authorities as well as the ones requested from the other courts and the law enforcement directorates, were classified. The socio-demographic features and psychiatric diagnoses of the cases were analyzed.RESULTS: Among 456 cases, 34.4% n=157 were female and 65.6% n=299 were male and the mean age was 38.33± 17.32. Of the cases, 56.8% of the forensic reports were of penal law while 32.7 n=149 , 12.3% n=56 and 1.8% n=8 were of civil law, status report and psychiatric diagnosis report requested from law enforcement directorates. CONCLUSION: In this study the higher incidence of schizophrenia and other psychotic disorders in cases evaluated in the context of crime law is noticable. In contrast to other studies from Turkey the lower incidence of alcohol and substance abuse is an important finding. Further studies investigating the distribution of diagnoses in forensic cases and its relation to crime may contribute to the establishment of preventive strategies.

Bir üniversite hastanesi psikiyatri polikliniğine başvuran adli olguların incelenmesi

Abstract

AMAÇ: Bu çalışmada psikiyatri polikliniğimizde değerlendirilen adli olguların geriye dönük olarak sosyodemografik özellikleri ve psikiyatrik tanıları, hangi amaçla adli psikiyatrik incelemeye yönlendirildiklerinin araştırılması amaçlanmıştır.YÖNTEM: Ocak 2012- Kasım 2015 tarihleri arasında üniversitemiz psikiyatri polikliniğine adli kurumlar tarafından gönderilen 18 yaş ve üzeri 456 olgunun tıbbi dosyaları ve düzenlenen raporları geriye dönük olarak incelenmiştir. Ceza hukuku, medeni hukuk, farklı mahkemeler ve emniyet müdürlüklerinden istenen raporlar ayrı ayrı sınıflandırılmıştır. Ayrıca olguların sosyodemografik verileri ve psikiyatrik tanıları incelenmiştir.BULGULAR: Çalışmamıza alınan 456 olgunun %34.4’ü n=157 kadın, %65.6’sı n=299 erkek olup, genel yaş ortalamaları 38,33± 17,32 bulunmuştur. Olguların %56.8’i n=259 ceza hukuku, %32.7’ si n=149 medeni hukuk, % 12.3‘ü n=56 durum bildirir, %1,8’i n=8 emniyet diliminin belirtilmesinin istendiği raporlar olarak tespit edilmiştir.SONUÇ: Çalışmamızda ceza hukuku kapsamında değerlendirilen olgularda şizofreni ve diğer psikotik bozukluk tanılarının daha yüksek oranda görülmesi dikkat çekicidir. Ayrıca ülkemizde yapılan diğer çalışmalardan farklı olarak değerlendirilen olgularda alkol ve madde kullanım bozukluğu tanısının diğer tanılara göre daha düşük oranda bulunması çalışmanın önemli bir sonucudur. Ülkemizde psikiyatri polikliniğine başvuran adli olguların tanı dağılımı ve suç ilişkisi ile ilgili yeterli veriler oluşturulması koruyucu ruh sağlığı politikalarının belirlenmesine ve suç işleme davranışının önlenmesine yardımcı olabilir.
