Abstract
Tabanca ile kafa bölgesine tek
atışla yapılan intihar girişimleri adli tıp açısından sıradan bir
durum olmasına rağmen, beyine
birden fazla atış oldukça nadirdir. Bunun nedeni ilk merminin
girdiği anda gelişen ani bilinç
kaybı ve sistemik kaslarda gevşemedir.
Penetran açık kafa travmasıyla
acil servise getirilen 28 yaşında,
erkek olgunun radyolojik incelemelerinde iki adet mermi çekirdeğinin kraniuma girmiş olduğu
saptandı. İntraserebral ve subdural kanaması düzeltilmesine
rağmen aynı gün olgu hayatını
kaybetti.
Olayın adli inceleme sonucu intihar olarak değerlendirildi. Mekanik bir sorunu olmayan yarı
otomatik bir tabancayla iki merminin patlaması ancak tetiğe iki
defa basılması ile gerçekleşeceği için, olay patofizyolojik ve literatür bilgileri ölçüsünde araştırıldı. Beyine iki adet merminin
girdiği, yarı otomatik silahla
gerçekleştirilen sadece 8 intihar
olgusu bildirilmiştir. Otomatik
silahlarla bile birden fazla merminin beyine isabet ettiği intihar
olgularının sayısı fazla değildir.
Beyine giren mermi sayısı olayın
cinayet-intihar ayrımında önemli bir ölçüt olması nedeniyle; birden fazla mermi ile gerçekleşen
intihar olayları balistik, kriminolojik ve patofizyolojik olarak ayrıntılı incelenmelidir. Yazımızda
intihar amacı ile kafaya yöneltilen tabanca ile çok nadiren de
olsa iki atış yapılabileceğini vurgulamak istedik.
