Yoğun Bakım Takibi Gerektiren Obstetrik Durumlar: Ankara Eğitim ve Araştırma Hastanesi Deneyimi

Abstract

Amaç: Hastanemiz kadın hastalıkları ve doğum kliniğinde tedavi
gören ve yoğun bakım takibi gerektiren obstetrik durumları özetlemek

Materyal-Metod: Kliniğimizde 2015-2018 yılları arasında
yatan gebeler retrospektif olarak incelendi. Bu dönemde yoğun bakımda tedavi
edilen 34 hastanın bilgileri elektronik data ve hasta dosyaları incelenerek
özetlendi.

Bulgular: Ocak 2015-Aralık 2017 arasında kliniğimizde 9110 doğum
gerçekleşti. Yatarak tedavi edilen hastaların 34 tanesinde yoğun bakım ihtiyacı
gelişti (%0,37). En sık yoğun bakıma yatış endikasyonları hipertansiyon (n=21,
%61,7) ve kanama idi (n=6, %11,8). Gelişen tek mortalite plasenta dekolmanına
bağlı postpartum kanama kaynaklıydı (n=1, %2,9). Yoğun bakımda tedavi edilen
hastalarımızda APACHE skorlaması ile öngörülen ölüm %9,9 iken gerçekleşen ölüm
oranı ise %2,9 idi.







Sonuç: Kliniğimizde obstetrik vakalar için yoğun bakıma yatma
oranı 1000 doğum başına 3,7’dir. Bu oran gelişmiş ülkeler benzerlik
göstermesine rağmen tüm ülkemizin durumunu yansıtan daha geniş çalışmalara
gereksinim vardır.
