IS ARTHROSCPIC REPAIR OF ROTATOR CUFF TEARS GOLD STANDART FOR WHOLE PATIENTS?

Abstract

OBJECTIVE: We aimed to investigate whether the arthroscopic repair is the first surgical choice or not for whole patients with rotator cuff tears in this study. MATERIAL AND METHODS: We retrospectively evaluated 102 patients who were operated for rotator cuff tears in our clinic. Patients who were operated by arthroscopic or arthroscopy associated mini-open repair were divided in two groups with regard to age as younger than 5th decade or older. Patient’s age, side, duration between beginnings of complaints to surgery, tears types, retraction levels, surgery times, used implant types and counts, comorbid diseases and cost of treatments were achieved from hospital records of patients. All surgical procedures were applied by same surgical team. At last follow-up, range of motion; measured with a goniometre and Constant shoulder scores were recorded for each patient. RESULTS: 39.2% of patients were under age of 50 while 60.8% were above. Mean duration of complaints were 10 months for entire of patients. Mean constant scores of all patients under age of 50 who were operated by arthroscopic or mini open surgery were over 80. For patients over age of 50; mean constant score of patients treated with arhroscopic repair were over 80 while patients treated with mini-open repair were under 65. CONCLUSION: Arthroscopic repair of rotator cuff tears is a gold standart method for patients over 50. No difference remains between arthroscopic or mini-open repair for patients younger than 50

ARTROSKOPİK TAMİR ROTATOR KAF YIRTIĞI HASTALARININ TAMAMI İÇİN ALTIN STANDART MIDIR?

Abstract

AMAÇ: Bu çalışmada amacımız rotator manşet yırtığı olan hastaların tamamında, ilk cerrahi tercihin artroskopik tamir olup olmadığını araştırmaktır.GEREÇ VE YÖNTEMLER: Kliniğimizde tam kat rotator manşet yırtığı nedeniyle ameliyat edilmiş 102 hasta, geriye dönük değerlendirildi. Artroskopik ya da artroskopi destekli mini açık tamirle tedavi edilen hastalar yaşlarına göre 5. dekat öncesi ve 5. dekat sonrası olarak iki gruba ayrıldı. Hastaların dosyalarından cerrahi sırasındaki yaşı, tarafları, ilk şikâyetten cerrahiye kadar geçen süreleri, yırtık tipleri, retraksiyon miktarları, ameliyat süreleri, kullanılan implant çeşidi ve sayıları, eşlik eden hastalıkları ve hastane maliyetleri kayıt edildi. Cerrahi tedaviler aynı ekip tarafından uygulanmıştır. Hastaların son kontrollerinde eklem hareket açıklığı goniometre ile ölçüldü, fonksiyonel değerlendirme Constant omuz skorlaması ile yapıldı.BULGULAR: Hastaların %39.2'si 50 yaş altındayken, %60.8'i 50 yaş üzerindeydi. Tüm hastaların şikâyet süresi ortalaması 10 ay olarak bulundu. Artroskopik ya da mini açık tamir yöntemiyle tedavi edilen 50 yaşından küçük tüm hastaların ortalama Constant skoru 80'in üzerindeydi. Bununla birlikte 50 yaşından büyük olup; artroskopik yöntemle tedavi edilen hastaların ortalama Constant skoru 80'den büyük iken, mini açık yöntemle tedavi edilenlerin hastaların ortalama Constant skoru 65'in altında bulundu.SONUÇ: Rotator manşet yırtığının artroskopik onarımı 50 yaşından büyük hastalarda altın standarttır. 50 yaşından küçük hastalarda ise artroskopik ve mini açık tamir sonuçları arasında fark yoktur
