RETROSPECTIVE ANALYSIS OF 310 PATIENTS WITH MALIGNANT SKIN TUMORS: EPIDEMIOLOGIC AND CLINICAL CHARACTERISTICS

Abstract

OBJECTIVE: Recently, skin tumors present an important amount when all types of cancers that can be detected on humans are concerned. Although some types of skin tumors can show a slow progress, there are also different types of tumors that can disseminate easily and cause mortality. In this study, it was aimed to analyze demographic and clinical features of the patients followed up with a diagnosis of skin tumor in our clinic and by this way, to call attention to this important subject. MATERIAL AND METHODS: In the study, 310 patients with skin tumors who were treated by surgeries under general anesthesia between 2000 - 2016 were evaluated retrospectively. Gender, age, tumor type, tumor localization, treatment method and excisional margin were used as evaluation criteria. RESULTS: It was detected that 174 of the patients 56% were male and 136 of them 44% were female. Average age of the patients was 64.1 years 30-92 years . Basal cell carcinoma was detected in 180 patients 58.1% and this was followed by squamous cell carcinoma in 104 patients 33.6% and malignant melanoma in 11 patients 3.5% respectively. Nasal region was detected as the mostly affected region 24.2% . As treatment modality, reconstruction with flaps was performed in 157 patients 50.6% and this was followed by reconstruction with full or split thickness skin grafts in 104 patients 36.8% , primary reconstruction in 21 patients 6.8% and reconstruction with both flap and grafts in 18 patients 5.8% . CONCLUSION: In conclusion, results of our study were in concordance with the literature findings. It is an important reality that must be kept in mind by all the clinical doctors that a great amount of skin tumors can be totally cured with an early diagnosis and treatment. We believe that this study can be valuable by reminding this frequent problem and by creating a source for met analysis type of studies that can be planned in a more comprehensive fashion

MALİGN CİLT TÜMÖRLÜ 310 OLGUNUN RETROSPEKTİF ANALİZİ: EPİDEMİYOLOJİK VE KLİNİK ÖZELLİKLER

Abstract

AMAÇ: Son yıllarda insanda tespit edilen tüm kanser türleri değerlendirildiğinde cilt tümörleri oldukça önemli bir oranı oluşturmaktadır. Her ne kadar bazı cilt tümörleri yavaş bir seyir göstermekteyse de hızlı yayılan ve mortaliteye neden olabilen değişik tümör tipleri de mevcuttur. Bu çalışmada da kliniğimizde cilt tümörü tanısıyla takip ve tedavisi yapılan hastaların demografik ve klinik özelliklerini incelemek ve böylelikle bu önemli konuya tekrar dikkat çekmek amaçlanmıştır. GEREÇ VE YÖNTEMLER: Çalışmada 2000-2016 yılları arasında genel anestezi altında opere edilerek tedavisi uygulanmış 310 cilt kanserli olgu retrospektif olarak incelenmiştir. Değerlendirme kriteri olarak cinsiyet, yaş, tümör tipi, tümör lokalizasyonu, tedavi yöntemi ve uygulanan eksizyon marjini kullanılmıştır. BULGULAR: Değerlendirilen hastaların 174’ü erkek %56 , 136’sı kadın %44 olarak bulundu. Hastaların ortalama yaşı 64.1 olarak tespit edildi 30-92 yaş . 180 hastada %58.1 bazal hücreli karsinom izlenirken bunu görülme sıklığı açısından sırasıyla 104 hastada %33.6 izlenen skuamöz hücreli karsinom ve 11 hastada %3.5 izlenen malign melanom takip etti. Nazal bölge en sıklıkla etkilenen bölge olarak tespit edildi %24.2 . Tedavi şekli olarak 157 olguda %50,6 flep ile onarım uygulanırken bunu sırasıyla; 104 olguda %36.8 uygulanan tam veya kısmı kalınlıkta deri grefti ile rekonstrüksiyon, 21 olguda %6.8 uygulanan primer onarım, 18 olguda %5.8 uygulanan flep ve greft ile onarım takip etti. SONUÇ: Sonuç olarak çalışmamızda elde edilen bulguların literatür bulguları ile uyumlu olduğu gözlendi. Cilt tümörlerinin büyük bölümünün erken tanı ve tedavi ile tamamen tedavi edilebildiği tüm klinisyenlerin aklında tutması gereken bir gerçektir. Çalışmanın hem bu sık görülen sorunu hatırlatması açısından hem de daha kapsamlı planlanabilecek olan meta analizlere kaynak oluşturması açısından değerli olduğu düşüncesindeyiz
