AKUT İSKEMİK İNME VE GEÇİCİ İSKEMİK ATAK GEÇİREN HASTALARDA MİZAÇ KİŞİLİK ÖZELLİKLERİ VE YAŞAM OLAYLARININ İNCELENMESİ

Abstract

Akut iskemik inme ve geçici iskemik atak geçiren hastalarda mizaç kişilik özellikleri ve yaşam olaylarının etkisi
ÖZET
AMAÇ: Akut iskemik inme ve geçici iskemik atak vasküler hasara bağlı olarak serebral kan akımının bozulması nedeniyle gelişir. İnme gelişiminde çeşitli genetik ve çevresel nedenler sorumlu olmakla birlikte çalışmalarda psikolojik faktörlerin de rol alabileceği saptanmıştır. Bu çalışmada kişilik özellikleri, mizaç ve geçirilmiş yaşam olaylarının inme ile ilişkili olup olmadığını saptamak amaçlanmıştır.
GEREÇ VE YÖNTEM: Çalışmaya NIH İnme Ölçeği puanı 10’nun altında olan, afazik olmayan, demans ve başka bilişsel bozukluğu olmayan 48 akut iskemik inme veya geçici iskemik atak geçiren hasta ile yaş ve cinsiyet uyumlu 47 gönüllü kontrol grubu dahil edilmiştir. Hasta ve kontrol grubuna sosyodemografik veri formu, Memphis, Pisa, Paris and San Diego Mizaç Değerlendirme Anketi, Mizaç ve Karakter Envanteri ve Yaşam Olayları Listesi uygulanmıştır.
BULGULAR: İnme hastaları ile kontrol grubunda yer alan sağlıklı bireyler arasında Memphis, Pisa, Paris and San Diego Mizaç Değerlendirme Anketinin depresif mizaç, irritabl mizaç ve anksiyöz mizaç alt ölçek puanları açısından istatistiksel olarak anlamlı fark saptanmıştır. Çalışma grupları arasında Yaşam Olayları Listesi ölçeğinin distres ve uyum alt ölçek puanları açısından istatistiksel olarak anlamlı bir fark saptanmamıştır. Mizaç Karakter Envanteri ise her iki grup arasında yenilik arayışı, zarardan kaçınma, sebat etme, kendi kendini yönetme ve iş birliği puanları açısından anlamlı farklılıklar saptanmıştır.
SONUÇ: İnme hastalarının depresyona eğilimli mizaç ve karakter özelliklerine sahip olduğunu tespit edilmiştir. Bu çalışma, ülkemizde inme geçiren hastaların mizaç ve karakter özellikleri ve yaşam olayları karşında verdikleri distres ve uyumu değerlendiren ilk çalışmadır.

Abstract

Effects of life events and temperament, personality traits in patients with acute ischemic stroke and transient ischemic attack
ABSTRACT
AIM: Acute ischemic stroke and transient ischemic attack develops due to impaired cerebral blood flow caused by vascular damage. Although various genetic and environmental causes are responsible for the development of stroke, it has been determined that psychological factors may also play a role in the studies. In this study, it is aimed to investigate whether personality traits, temperament and past life events are related to stroke.
MATERIAL AND METHODS: The study included 48 patients with a National Institues Of Healt Stroke Scale scores below 10, non- aphasic, without dementia and other cognitive impairments and 47 patients with age and gender matched control groups. Sociodemographic data form, Temperament Evaluation of Memphis, Pisa, Paris and San Diego Autoquestionnaire, Temperament and Character Inventory and Life Events Scales were applied to the patient and control groups.
RESULTS: A statistically significant difference was found between stroke patients and healthy individuals in the control group in terms of Temperament Evaluation of Memphis, Pisa, Paris and San Diego Autoquestionnaire scale, depressive temperament, irritable temperament and anxious temperament subscale scores. There was no difference between the study groups in terms of the distress and adjustment subscale scores of the Life Event Scale. In the Temperament and Character Inventory scoring, significant difference was found between two group in terms of seeking innovation, avoiding harm, persistence, self-directedness, and cooperation.
CONCLUSİON: It has been determined that stroke patients have temperament and characteristics that are prone to depression. This study is the first study to evaluate the distress and harmony of patients who have had a stroke in our country against their temperament and character characteristics and life events.
