Nursing Care of the Frail Elderly: A Case Report Based on the Empowerment Model

Abstract

This study aimed to evaluate the effects of an empowerment model–based nursing care on health outcomes in a frail older adult. The study was conducted as a qualitative case study involving an older patient with severe frailty and multiple geriatric syndromes following stroke. Individualized nursing interventions were planned and implemented in accordance with the empowerment model. At the end of the care process, improvements were observed in the management of incontinence, medication adherence, sleep regulation, physical activity tolerance, and fall risk. The findings indicate that the empowerment model provides a systematic, holistic, and feasible framework for nursing care in frail older adults.

Kırılgan Yaşlının Hemşirelik Bakımı: Güçlendirme Modeli Temelli Olgu Sunumu

Abstract

Bu çalışmanın amacı, güçlendirme modeline dayalı hemşirelik bakımının kırılgan yaşlı bireylerde sağlık sonuçları üzerindeki etkisini değerlendirmektir. Çalışma, inme sonrası çoklu geriatrik sendromlar ve şiddetli kırılganlık düzeyi bulunan bir olgu üzerinden yürütülen nitel bir olgu çalışmasıdır. Güçlendirme modeli doğrultusunda bireyselleştirilmiş hemşirelik girişimleri planlanmış ve uygulanmıştır. Bakım süreci sonunda olgunun inkontinans yönetimi, ilaç uyumu, uyku düzeni, fiziksel aktivite toleransı ve düşme riski açısından olumlu gelişmeler gözlenmiştir. Elde edilen bulgular, güçlendirme modelinin kırılgan yaşlı bireylerde hemşirelik bakımına sistematik, bütüncül ve uygulanabilir bir çerçeve sunduğunu göstermektedir.
