Sezaryen İle Doğuma Güncel Bir Bakış: Modern Sezaryen Teorisi

Abstract

Doğum, tüm toplumlarda sağlık sisteminin en hassas ve en önemli indekslerinden biridir. Bu nedenle hem vajinal yolla hem de sezaryen ile doğum yapacak herkese sunulan bakım; kanıta dayalı, sağlığı iyileştirici ve düşük maliyetli olmalıdır. Sezaryen ameliyatı, maternal ve fetal sağlık için hayat kurtarıcı bir müdahale olmakla birlikte cerrahi bir girişimdir, sağlığı kısa veya uzun süreli etkileyebilir ve maliyeti vajinal doğuma göre daha yüksektir. Buna rağmen günümüzde sezaryen oranlarında önemli ölçüde artış görülmektedir. Modern sezaryen teorisine göre bu artışın yaşanmasında sağlık ve teknoloji alanında yaşanan gelişmelerin etkisi oldukça büyüktür. Aynı zamanda bu teori, kendi bedenine güvenmeyen, sezaryenin bebeğinin sağlığı için en uygun seçenek olduğuna inandırılan kadınların daha düşük olasılıkla vajinal doğum yaptığını belirtmekte ve bu konuda en büyük sorumluluğun ebelere düştüğünü savunmaktadır. Ebeler, prekonsepsiyonel dönemden itibaren kadın ve ailesinin gereksinim duyduğu tüm alanlarda bilgilendirilmesini sağlamalıdır. Ayrıca yasa ve politikalar ile ebelerin görev ve sorumlulukları net bir şekilde belirlenmeli ve vajinal doğumun sorumluluğu ebelere bırakılmalıdır.

A Current Overview of Cesarean Birth: The Modern Cesarean Theory

Abstract

Birth is one of the most sensitive and important indexes of the health system in all societies. For this reason, the care offered to everyone who will give birth both by vaginal delivery and by cesarean section; It should be evidence-based, health-promoting and cost-effective. Although cesarean section is a life-saving intervention for maternal and fetal health, it is a surgical intervention, it can affect health in the short or long term, and its cost is higher than vaginal birth. Despite this, there is a significant increase in cesarean section rates today. According to the modern cesarean section theory, the effects of developments in health and technology in this increase are quite large. At the same time, this theory states that women who do not trust their own bodies and who are convinced that cesarean section is the most appropriate option for their baby's health are less likely to give birth naturally, and argues that the biggest responsibility in this regard falls to midwives. Midwives should ensure that women and their families are informed in all areas they need from the preconceptional period. In addition, the duties and responsibilities of midwives should be clearly defined with laws and policies and the responsibility of vaginal birth should be left to midwives.
