Yoğun Bakım Hastalarında Göz Komplikasyonları ve Göz Bakımı

Abstract

Oküler yüzey bozuklukları yoğun bakım hastalarında sık karşılaşılan sorunlardan birisidir. Özellikle yaşamsal organlardaki bozuklukları tedavi etmek, organ yetmezliklerinin yönetimini sağlamak üzerine odaklanılan yoğun bakımlarda göz bakımı konusu daha az önemsenmekte ve göz ardı edilebilmektedir. Yoğun bakım hastasında bozulan bilinç, mekanik ventilatör desteği ve ventilatör desteğinin devam etmesi için kullanılan ilaçlar oküler yüzeyin hassasiyetini arttırmaktadır. Korneanın epitel bozuklukları, kemozis, keratit ve kuru göz gibi komplikasyonlar yoğun bakımlarda en sık karşılaşılan sorunlardır. Yoğun bakımlarda göz bakımı uygulamaları henüz standart bir şekilde gerçekleştirilmemekte, göz bakımına ilişkin uygulamalar çeşitlilik göstermektedir. Bu nedenle yoğun bakım hastalarında göz bakımına yönelik rehberlerinin oluşturulması, standart göz bakımının verilmesini kolaylaştıracaktır. Standart göz bakımının verilmesi göze ilişkin komplikasyonların önlenmesinde etkili olacaktır. Bununla birlikte, yoğun bakım hastaları için tek tip kanıta dayalı göz bakımı protokolü geliştirmek için daha fazla araştırma yapılması gereklidir. Göz sağlığı, hastanın hayatta kalması tehlikede olsa bile, öncelikli konulardan biri olmalıdır. Bu nedenle, göz muayenesi ve bakımı yoğun bakım ortamında rutin değerlendirmenin bir bölümünü içermelidir. Bu derleme yoğun bakım hastalarında karşılaşılan göz problemlerine dikkat çekmeyi ve uygulanacak bakımın önemini vurgulamayı hedeflemektedir

Eye Complications and Eye Care in Intensive Care Patients

Abstract

Ocular surface disorders are more commonly seen problems in intensive care patients. Eye care is considered less important and can be ignored especially in intensive care units where it is focused on treating disorders in large organs and managing organ failure. Impaired consciousness, mechanical ventilator support, and medications required for maintaining ventilator support in intensive care patients increase ocular surface sensitivity. Complications such as corneal epithelial disorders, chemosis, keratitis, and dry eye are the most common problems in intensive care patients. Eye care practices in intensive care have not yet been performed in a standard manner, and eye care practices vary. In particular, there are significant differences in eye care practices among intensive care units. For this reason, the formation of eye care guidelines in intensive care patients will facilitate the provision of standard eye care. Providing standard eye care will be effective in preventing complications of the eye. However, further studies are needed to develop a uniform evidence-based eye care protocol for intensive care patients. Eye health should be one of the priority issues, even if the patient's survival is in danger. For this reason, eye examination and care should be a part of the routine assessment in the intensive care setting. The aim of this review is to draw attention to the eye problems encountered in intensive care units and to emphasize the importance of the care to be administered
