The evaluation of the late complications of the femoral neck fractures treated with the sliding hip nails

Abstract

62 patients with femoral neck fractures treated either with Richards Sliding nail "Richards compression screw" (135°) or Massie sliding nail (150°) between 1977 and 1987 in Department of Orthopaedics and Traumatology of Istanbul Universty Medical Faculty Hospital were studied with respect to late complications. 43 of the 62 cases were assessed with respect to union. Union was achieved in %73.9 of the 23 cases treated with Richards cmopression screw. Union rate was %100 in 20 cases treated with Massie sliding nail. Avascular necrosis was seen in % 30.8 of the 13 cases treated with the former while 16 cases treated with the latter revealed on avascular necrosis rate of %25. We concluded that, with the Massie silding nails which has a more appropriate angle as regards the load bearing axis, the load acting on the hip will create a persistent silding affect on the nail and thus the ongoing compression at the fracture site will enhance the union rate. Since the angle of the load acting on the femoral head and the angle between the nail and the side plate are almost identical, with the Massie sliding nail the load acting on the hip joint will results in persistent compression forces. The two nails didn’t show a significant difference as regards the rate of avascular necrosis. The cases which resulted with avascular necrosis were studied individually and the possible factors were investigated.
