The surgical treatment of lumbar spinal stenosis

Abstract

80 patients suffering from lumbar spinal stenosis have been treated in Universty of Ankara, Medical Faculty, Orthopaedics and Traumatology Department, and some several hospitals l978 and l988. The distribution of the patients is as follows; a) 10 cases vith lateral spinal stenosis. b) 14 cases vith central mixt stenosis. c) 32 cases that have developed spinal stenosis after laminectomie or fusion. d) l6 cases that have spinal senosis combined with degenerative scoliosis. The CT scaning has been used in deciding the level of operation. However in some 6 patients, that have been operated before, the CT scaning has showed to be fooling. For those patients for the ones who had lateral spinal stenosis, unilateral laminectomie and partiel fasectomie, and for the 28 cases who had centrol mixt stenosis, decompressive laminectomie and bilateral fasectomie have been performed. For the 9 cases who had scoliosis and spinal stenosis, spinal fusion at the concex side, and partiel fasectomie with laminectomie at the concave side have been performed. The 32 patients that have developed spinal stenosis after laminectomie and spinal fusion are the ones that have showed difficulty in evaluation. In this group there are 19 good, 8 fair and 5 poor results. From the 80 cases in 57 (%7l), we have had good results after sugical procedures.

Lumbar spinal stenosis in cerrahi tedavisi

Öz

A.Ü.Tıp Fakültesi Ortopedi ve Travmatoloji Kliniği ve özel hastanelerde 1987-1988 yılları arasında lumbar spinal stenosisden muzdarip 80 hasta tedavi edilmiştir. Bunlar Lateral Spinal Stenosis 10 vak a. Central Mixt Stenosis 14 vak a, Laminektomi Füzyondan sonra gelişen Spinal Stenosis 32 vak a ve dejenaratif scoliosis ile müterafık Spinal Stenosisli 16 vak a olarak gruplandırılmıştır. Bu vak alarda müdahale edilecek seviyenin tayininde CT tetkiki çok yardımcı olmuştur. Maamafih daha önce birkaç ameliyat geçirmiş 6 vak ada CT Scan yanıltıcı olmuştur. Söz konusu vak alardan Cateral Spinal Stenosis tanımlanan hastalarda unilateral laminektomi ve parsiyel fasetektomi, Central Mixt Stenosisli 28 hastada dekompresiv laminektomi ve ilateral fasetektomi uygulanmıştır. Scoliosis ile müterafık Spinal Stenosisli 9 vak’ada konveks spinal füzyonla birlikte konkav tarafta parsiyel fasetektomi laminektomi yapılmış. Daha önce laminektomi ve spinal füzyon ameliyatından sonra spinal stenosis gelişmiş 32 hasta, analizleri en güç grubu teşkil etmiştir. Bu grupta, l9 iyi, 8 orta ve 5 kötü sonuç alınmıştır. Toplam 80 vak adan 57 sinde (%7l) cerrahi müdahaleden iyi sonuçlar istihsal edilmiştir.
