Treatment of congenital hip dislocation in juvenile and adolescent ages

Abstract

During the period 1960 to 1988, 3000 operations were performed in patients with congenital dislocation of the hip. Of these 784 patients were between 1 1/2 and 4 years of age, 1646 patients were between 4 and 8 570 patients were between 8 and 14 years of age with an average of 10 years. The procedure is perfomed in a single operation (Radical Reduction) by sectioning all contracted muscles, opening and debriding the joint and reducing the femoral head into the acetabulum after removal of a 2 cm to 3 cm bone segrnent from the subtrochanteric region of the femur. Fixation of the femoral shaft in a derotational varus position is done with a hip plate. Fınaliy a transiliac acetabuloplasty is performed. The bone segment removed from subtrochanteric region is used to support the acetabular roof. ln those cases where the cartilage has closed we performed a dome-type operation that we have developed. In case with complete dislocation a horizontal osteotomy of 1Ox50 mm, is cut from the spina iliaca anterior inferior to somewhere near the sciotic notch; 3 cm wide and 5 cm long peace of bone which is removed from iliac wing is tightly placed into the engroved site. To that an acetabular dome is perfomed immediately above the capsule. ln cases of subluxation or redislocation, a Chiari pelvic osteotomy is performed as the second operation.
