Evaluation and treatment in lateral epicondylitis

Abstract

This study have been done in 28 cases of lateral epicondylitis; 17 of them were females (61%) and 11 of them were males (39%) and mean age was 43.9±6. In 14 cases there was pain with palpation in C5-6 and/or C6-7 apophyseal joints of the cervical degenerative pathological findings in all of these patients cervical X-Rays. 17 patients out of 28, have been continued to therapy. II patients who had pain in cervical spine besides elbow pain were treated by 15 sessions of Ultrasound (1 watt/cm2 para vertebral, 5 minutes) and Infrared (20 minutes) and steroid injection was appliedlocally to the lateral humeral epicondyle. Local steroids were injected to 6 patients with elbow pain only, 40 mg. methylprednisolone has been used for local injection and injection has been repeated, for recurrence of pain, on no more than there occasions. The difference between the results of evaluation before and after the treatment was found to be statistically significant. (P<0.001) 88% of the patients responded very well to treatment and had complete cure. In 12% there was partial improvement. Cervical factor should be taken into consideration for the evaluation of lateral epicondylitis. Management of the underlining pathology would increase the success of the treatment results.

Abstract

Bu çalışma, 17'si kadın (%61); 11'i erkek (%39) 28 lateral epikondilitli olguda yapıldı. Yaş ortalaması 43.9±6 idi. 14 'ünde (%50) servikal omurgada-epikondilit ile aynı tarafta C5-6 ve/veya C6- 7 apofizer eklemlerin palpasyonu ile ağrı, bunların radyolojik tetkikinde tümünde aynı düzeylerde dejeneratif değişiklik saptandı. 28 olgunun 17'si tedaviye alındı. Servikal omurgada ağn saptanan 11 olguya (1. grup) servikal bölgeye 15 seans ultrason (paravertebral, 1 watt cm2 5 dakika) ve infraruj (20 dakika) ve epikondile lokal steroid injeksiyonu; epikondilde ağrısı olan 6 olguya (2. grup) yalnız lokal steroid injeksiyonu uygulandı. Lokal steroid enjeksiyonu birer hafta aralıklarla 40 mg. metilprednisolon (Depo medrol) kullanılarak yapıldı. Ortalama enjeksiyon sayısı 1. grupta 1, 2. grupta 1.8 idi. Tedavi öncesi ve sonrası testlerle değerlendirildiğinde aradaki farklılık istatistiksel olarak anlamlı bulundu. (P<0.001) Olguların %88'inde tam, %12'sinde kısmi iyileşme saptandı. Lateral epikondilit değerlendirilmesinde servikal faktörün gözönünde tutulması ve tedavinin buna göre yönlendirilmesi tedavide başarı oranını arttıracaktır .
