Wringer injuries of the upper extremity

Abstract

Such crushed injuries as those caused in upper extremities which pass between the cylinders of a washing machine are referred to as "wringer injury". The slightly damaged appearance of the superficial tissues observed during the first hours after the injury is frequently misleading. Care and early precautions which should be taken in order to prevent serious sequ.elae that might develop in the future are of utmost importance. In this report of ours, attempts were made to separate in two groups as early and late applications those 58 cases who referred to our clinic with wringer injuries during 1987-1990, and to discuss, under the light of literature, the mechanism of injury, its pathology, and the clinical findings of the patients their mode of treatment and rare of survival. When patients whith such type of injuries were hospitalized during the first 48 hours and followed with early drainage and decompression, adequate wound care, tissue necrosis was prevented and hand functions of the patients were maintained.

Üst ekstremitede merdane yaralanmaları

Öz

Üst ekstremitenin çamaşır makinası silindirleri arasından geçmesi ile oluşan ezici yaralanmaya "merdane yaralanması "denir. Yaralanmadan sonraki ilk saatlerde yapılan muayenede yüzeyel dokuların hafif hasarlı görünümü çoğu kez yanıltıcıdır. Dikkatli gözlem ve erken tedbirlerin alınması, ileride gelişecek olan ciddi sekellerin önlenmesi için çok önemlidir. 1987-1990 yıllan arasında merdane yaralanması ile kliniğimize başvuran 58 olgu, erken ve geç başvuranlar olmak üzere iki gruba ayrılmış; yaralanmanın oluş mekanizması, patolojisi, hastaların klinik bulgu, tedavi ve sürvilerinin karşılaştınlmalı analizinde, erken drenaj, dekompresyon ve uygun yara bakımı ile takip edilen hastalarda doku nekrozlan önlenip, el fonksiyonlanmn korunduğu gözlenmiştir.
