Arthroscopic diagnosis and treatment of the osteochondritis dissecans of the knee joint

Abstract

The results of treatment of osteochondritis dissecans of the knee joint in eight patients are evaluated in a prospective study. Arthroscopic in-situ forage was performed in 4 grade 2 lesions, arthroscopic curettage and fixation of the fragment via arthrotomy in one grade 3 lesion and excision of the fragment and forage of the crater in 3 grade 4 lesions. The results were evaluated according to Hughston's criteria and six excellent, one good and one fair result were obtained. The fair result is thought to be due to the accompanying ACL laxity in the patient. We concluded that our arthroscopic treatment protocol in 0. D. of the knee joint yields satisfactory results, especially in patients without any accompanying lesions of the knee joint.

Diz eklemi osteochondritis dissecansında artroskopik tanı ve tedavi

Abstract

Sekiz diz eklemi osteochondritis dissecans'ı (0. D.) vakasında uyguladığımız artroskopik tedavi protokolu ve elde edilen sonuçlar, prospektif olarak değerlendirildi. Grade 2 lezyonu olan 4 hastada in-situ artroskopik foraj, grade 3 lezyonu olan 1 hastada artroskopik küretaj sonrası artrotomi ile tespit, grade 4 lezyonu olan 3 hastada ise, fragmanın eksizyonu ve kratere foraj uygulandı. Sonuçlar Hughston kriterlerine göre değerlendirildi. 6 hastada çok iyi, 1 hastada iyi ve 1 hastada da orta sonuç alındı. Orta sonuç alınan hastada lezyonun beraberinde ön çapraz bağ instabilitesi bulunmasının sonucu etkilediği düşünüldü. 0.D. tanı ve tedavisinde, artroskopik tedavi protokolumuzun, diz içindeki diğer patolojiler nedeniyle komplike olmayan vakalarda başarılı olduğu sonucuna varıldı.
