Arthroscopically assisted closed lateral capsular release in the treatment of patellar instability and anterior knee pain syndromes

Abstract

Lateral capsular release (LCR}, popularized in 1977 by Ficat and Hungerlord is an arthroscopically assisted procedure perlormed in the treatment of patellar instabiltty lateral patellar hyperpression and anterior knee paın syndromes Twenty two LCR procedures were pel1ormed by the senior author between 1988-1990 The results of 14 patients who could be located for a follow-up visit were prospectively analyzed The study group consisted of 15 knees of 14 patients with a mean age of 33 3 years (16-58} and a mean lollow.up of 8. 8 months (4-28) The diagnosis was anterior knee pain in 3 lateral patellar hyperpression in 4, patellar subluxation in 4 and patellar-lemoral osteoarthn"lis in 4 knees The Iower extremity morphotype, joint laxlty, patellar gliding, T-sulcus angle, patellar mobility, lateral capsular contracture and vastus medialis bulk were eva!uated during the clinical examination. Patello-lemoral relationship was analyzed in the frontal, horizontal and sagittal planes with radiographs using the techniques described by Merchant, Caton, Laurin, and Maldaque-Malghem A diagnostic arthroscopy was perlormed and concomittant pathologies werf treated, followed by an intra or extra synoviallateral capsular release The resloration of patello-femoral congruency was verilied by radiographs and an intraarticular drain was inserted followed by a compressive dressing and ice-packs Conlinious passive motion was instituted in the first 24 hours and the patients were disharged on the fourth postoperative day There were no inlections or hematomas. The results were graded using the scoring system of hall. The results were rated as excellent in 2 and good in 1 knee in the anterior knee pain group; good in 3 and fair in 1 knee in the patellar subluxation group; excellent in 3 and good in 1 knee in the lateral patellar hyperpression group; and good in 1. fair in 1 and bad in 2 patients in the patello-lemoral osteoarthritis group it was concluded that arthroscopically assisted lateral capsular release is a simple and elleclive procedure when perlormed in patients who meet the indication criteria.
