Synthetic ligaments
Abstract
In an attempt to improve the results after autogenous anterior cruciate ligament (ACL) reconstruction, and to overcome the difficulties encountered after failed cases, Orthopaedic surgeons have been searching for new alternatives. Thus syhthetic ligaments and allografts evolved. These new alternatives were previously alIuded to by the author. In the present review, technical aspects of prosthetic reconstruction are discussed and new synthetic materials are added. These devices are very technique sensitive. In contrast to biological substitutes, technical errors are not tolerated by these devices. It should be noted that the same technical principles are valid for an ideal autogenous reconstruction. It is vital that the Orthopaedic surgeon dealing with prosthetic reconstruction has all the necessary instruments. Synthetic Iigaments Iike allografts, are still in the experimental stage, and their indications are Iimited. After informing the patient in detail about all types of reconstructions, prosthetic devices can be used if (s)he prefers them.

Başarısız ön çapraz bağ (ÖÇB) rekonstrüksiyonlarından sonra karşılaşılan güçlükler ve primer operasyonlarından daha iyi sonuçlar alma çabası ortopedistleri yeni arayışlara yöneltmiştir. Bu şekilde sentetik bağIar ve allogreftler kullanıma girmiştir. Bu konudaki genel bilgiler daha önce iki ayrı yazıda verilmişti. Bu yazıda daha çok sentetik bağlarla ilgili teknik özellikler üzerinde duruldu ve yeni sentetik materyallerden kısaca söz edildi. ÖÇB protezlerinin uygulanmasında teknik son derece önemlidir. Otojen dokular bazı teknik hataları nispeten tolere edebilmelerine rağmen, bu durum sentetik materyaller için geçerli değildir. Teknik özellikleri yerine getirmek için gerekli olan aletlerin tümü ortopedistin elinde olmalıdır, aksi halde bu tür bir ameliyata girişilmemelidir. Sentetik bağların kullanımı halen deneysel olup endikasyonları sınırlıdır. Hastaya tüm rekonstrüksiyon işlemleri ile ilgili ayrıntılı bilgi verildikten sonra tercihine göre endikasyon sınırı genişletilebilir.
