The management of derotational effect of Cotrel-Dubousset instrumantation with computerized axial tomography

Abstract

In the pathogenesis of idiopathic scoliosis vertebral rotation in the axial plane is thought to be the primary deformity according to various opinions. In this rotation degrees of apical vertebras are evaluated by computed tomography in 35 flexible thoracal lordscoliotic patients, in whom derotation manoeuvre is performed Cotrei-Dubousset instrumentation technique between December 1988 and February 1991 at the ist orthopaedics and Traumatology Clinic of Ankara Social Security Hospital. Preoperative RAsag was 27.5, RAml was 52.2 and RAdev was 24.7 and correction rates were 23.5%, 247% and 15.7% respectively in the postoperative period. Furthermore, correction rate was found to be correlated with the correction rates in rib hump deformity. ln view of these data, it is suggested that Cotrel-Dubousset technique not only has success in correction of deformities in the frontal and sagittal planes but decreases vertebral rotation aswell.
