The morphologic and radiologic evaluation of the acromioclavicular joint in healthy individuals

Abstract

The topographic and functional anatomy and the age depended osteoarthrosis of the right acromioclaviculer joint has been examined on 39 dead bodies by means of anatomopathologic and radiologic tools. The AC joints have been extirpated in to, fixed in formalin and pinned in the horizontal plane; they have been arthrotomizedn and the macroscopic view of the joint surface photographed. Subsequently, the joint surfaces have been lamellized in the frontal plane, and decalcified; three different histologic slices have been examined and the arthrosis rate determined after OTTE. In addition, an arthography has been performed in maximal abduction and adduction position on 11 shoulders. The articulare surfaces and the disc of the AC-joint show a wide range in their shapes, sizes, congruence and mobility. The first signs of arthrosis are determined histologically beginning with the age of 20 and radiologically after the age of thirty. In same cases, the joint surface of the clavicle reveals a higher rate of chondral destruction. The early onset of the arthrosis of the AC joints is explained by the high mechanical loading of the relatively small articulare surface by rotationel, sliding and wedging motions, and the poor chondral coating of the joint.
