Antibiotic prophylaxis for open fractures
Abstract
In this review article, antibiotic prophylaxis for open fractures are considered. The risk of infection in acutely traumatized patients with open fractures is considerable Subeguent complications may include chronic sepsis (10-24%), delayed or failed union of the fracture (9-23%), functionalloss (50%), amputation (18%) and deformity, often resulting in prolonged and repeated hospitalizations. Success or failure in preventing infection depends to a great extent on appropriate surgical management, such as adeguate debridement, irrigasyon, and, if possible, primary skin closure. There are several identifiable sources of infection, however, that may be beyond the control of the surgeon. For example, traumatic injuries are freguently contaminated with organisms that colonize the skin or are introduced during injury. In fact, 70% of open fractures are contaminated at the time of traumaa. Staphylococcus is the most common infecting organism, although other gram ppositive and gram negative organisms may be found. In addition, malnutrition due to post-injury hypermetabolism may also supress inflammatory responses and cell-mediated immunity. In recent years, antibiotic prophylaxis has been used as an adjunct to appropriate surgical management to prevent infections in open fractures. Although there are many antimicrobial agents available, there are few guidelines for assessing their value in patients surgically treated for open fractures. This article will review the published literature on antibiotic prophylaxis in open fractures and examine current practices andoptions available to the physician. The suitability of regimens with a broader spectrum will be discussed in this article.
