Surgical management of adult femoral shaft fractures, and the place of the interlocking intramedullary nalling in this management

Abstract

Aduft femoral shaft fractures and their treatment are the important issues in traumatofogy. These fractures are grouped as stable and unstable adult femoraf shaft fractures. Stable adult femoral shaft fractures are treated satisfactorily with conventional intramedullary nailing. In our series 2 of the 15 cases of stable adult femoral shaft fractures were treated with plating 13 of them were treated with conventional intramedullary nailing. In spite of poor results in two cases for which osteosyntesis by plating was performed we achieved 85 percent excellent or good results in cases which were treated with conventional intramedullary nailing. Recently use of interlocking intramedullary nailing has been gradually increased in the treatment of unstable adult femoral shaft fractures. We also used this method in 20 of 35 cases with unstable adult femoral shaft fractures, and various plating techniques were used in the rest of them. We achieved 54 percent excellent or good results in cases which were terated with plating and 85 percent excellent or good resufts in cases which were treated with interlocking intramedullary nailing. Consequently, we recommend conventional intramedullary nailing in patients who have unstable aault femoral shaft fractures as a safe surgical method.
