The examination of nosocomial infections that develop in the orthopedics clinic
Abstract
In this study we aimed determine the hospital infection incidents, the active microorganisms and the resistance of these against antibiotics in our clinic. During the period of 1. 3. 1991-1. 1. 1993 the patients who were treated in the orthopedics clinic were observed daily and the incidents of hospital infections were recorded. In addition, their active microorganisms were determined; and then the resiistance of against antibiotics were determined by the disc diffusion method. In our study while the rate of hospital infections was found to be 7.2%, that of the operative injury infections took the Iead with a rate of 74.4%, 60.2% of the active microorganisms were gram negative bacterias and the most important ones among them were Pseudomonas and Enterbacter types. Staphylococcus aureus and Staphylococcus coagulase negative were the most common gram positive bacterias. The resistance of gram negative against penicillines containning ampicillin, amoxillin, mezlocillin and piperacillin werefound to be above 83.8%. Their resistance against gentamicin from the aminoglycoside was 66.1 % and against netilmicin the rate was 25.3%. Additionally, their resistance against ceoeperazone was 64% and against cefotaxime it was 58. 7%. Gram positive bacterias were observed to be resistant against the penicillines containning penicilline G, ampicilin, amoxicillin, mezlocillin and piperacilline with the rates 64.4%-91.1 %. While their resistance rates against sulbactam-ampicillin, clavuland-amoxicilline were 38%, 29.1% respectively, for cefalosporins this rate was found to be between 28.8% and 46.6%. The most effective antibiotics for gram negative and positive bacterias were ciprofloxacin and ofloxacin. The resistance rates of these were 14% and 19.3% respectively. As a result the most often isolated bacterias were Pseudomonas, Enterobacter, Staphylococcus coagulas negative and staphylococcus aureus. it was observed that among the hospital infections operative injury infections operative injury infections took the Iead and bacterias were highly resistant against the antibiotics.
