Discoid lateral meniscus and arthroscopic surgery

Abstract

Instead of having a semilunar structure, the circular appearance of the lateral meniscus is described as a discoid meniscus. In this clinical evaluation of our arthroscopic experience, we aimed to review the incidence and the Iong-term therapeutic results of this rare condition. Among the 5400 arthroscopic examinations performed in the knee joint either for diagnosis or for therapy in our clinic, between December 1983 and December 1993, 73 cases (1.35%) (30 female, 43 male and 37 right, 361eft) have been proven to have lateral discoid menisci. Additional degenerative or traumatic lesions in the discoid menisci have been shown in 30 of the 39 cases whose complaints in the knee region were thought to be related to the presence of a lateral discoid meniscus. In all symptomatic cases, reconstruction using partial meniscectomy, trimming and shaving have been performed, whereas no surgical interventions were intended in asymptomatic ones. In 7 of the surgically treated cases, effusion in the knee joint was observed within the first week postoperatively and responded well to antiinflammatory therapy. During the 3-month follow-up period, 15.3% of the cases (6 cases) exhibited knee pain, subjective instability and atrophy of the quadriceps muscle, while this ratio decreased to 3.3% (1 case) within the 6-month follow-up. As a conclusion, due to its minimal traumatic effects, opportunity for early mobilization, a Iower incidence of complications and satisfactory results, the arthroscopic surgical approach should be the preferred treatment modality for those cases in which surgery is indicated.
