The patellofemoral joint in total knee arthroplasty

Abstract

We compared two groups of patients, one with and the other without replacement of the pateflar surface, to study the relation of anterior knee pain after total knee arthroplasty with patellar sutface replacement. There was anterior knee pain in 8.7% of 23 knees with pateflar surface replacement and in 23.7% of 59 knees without patellar surface replacement. There was difficulty in functions regarding the patellofemoral joint, such as ascending and descending the stairs and standing up from a chair, in 13% of the replaced knees and 27% of the non-replaced knees. On statistical analysis the difference between the two groups were not statisticafly significant. Although statistically insignificant. the rate of patellofemoral pain is less in patients who had patellofemoral surface replacement. Therefore, we prefer to replace the patellofemoral surface paying special attention to patellofemoral tracking and protection of patellar circulation.

Total diz artroplastisinden sonra karşılaşılan dizönü ağrısının patellar komponent değişimi ile ilişkisini araştırmak amacıyla patellar yüzeyi değiştirilen ve değiştirilmeyen iki grup hasta karşılaştırılmıştır. Patellar yüzeyi değiştirilen 23 dizin %8.7 sinde, değiştirilmeyen 59 dizin ise %23.7 sinde dizönü ağrısı saptanmış, değiştirilen dizlerin % 13 ünde, değiştirilmeyenlerin %27'sinde merdiven inip çıkma ve iskemleden kalkma gibi patellofemoral eklemi ilgilendiren fonksiyonlarda güçlük görülmüştür. Sonuçların istatistiki analizinde her iki grup arasındakı farklılıklar anlamlı bulunmamıştır. İstatistiksel olarak anlamlı olmamakla birlikte patellofemoral yüzeyi değiştirilen hastalarda patellofemoral ağrı oranı daha düşüktür. Bu nedenle patellofemoral tracking ve patellar dolaşımın korunması gibi teknik özelliklere dikkat edilerek patellofemoral yüzeyin değiştirilmesini tercih etmekteyiz.
