Ilizarov external fixation method for treatment of congenital tibia pseudoarthrosis

Abstract

Basic treatment principle for congenital tibia pseudoarthrosis (CTP) is to provide solid union and to obtain a functional extremity. Classical treatment methods fail to treat both pseudoarthrosis and limb lenght discrepancies in one cession, and frequently a second operation for limb lengthening is needed. By using Ilizarov External fixation method, treatment of pseudoarthrosis and correction of deformity along with limb lengthening procedure in a single operation cession is possible. Five patients (3 boys, 2 girls; mean age 8.3 years) diagnosed as having CTP were operated in SSK Istanbul Hospital 2nd Orthopacdicsand Traumatology Clinics from February 1991 to March 1994, and Ilizarov extemal fixation method was used for the treatment of CTP. Three patients followed average 32 months were included in the study. According to Polen's Criteria, results were excellent for two patients, and good for one. Solid union of the extremity was detected approximately within 7.5 months and average 3 centimeters of limb lenghening was performed. These treatment results of CTP are satisfactory both for the patient and for the surgeon since solid union of the extremities without length discrepancies are the goalds of the treatment which are easily obtained by Ilizarov external fixation method. Nevertheless the number of series should be increased to be able to discuss this method in a broad sense.

Konjenital tibia pseudoartrozunun tedavisinde İlizarov yöntemi

Abstract

Konjenital tibia pseudoartrozanda tedavideki temel prensip kaynamanın sağlanması ve fonksiyonel bir ekstremitenin elde edilmesidir. Tedavide kullanılan, çeşitli cerrahi tekniklerle kaynama sağlanabilmekte, mevcut bulunan kısalık ise ayrı bir tedaviyi gerektirmektedir. Bu nedenle İlizarov yöntemi patolojinin tüm öğelerini aynı seansta düzelten bir yöntem olarak karşımıza çıkmaktadır. SSK İstanbul Hastanesi 2. Ortopedi ve Travmatoloji Kliniği'nde Subat 1991-Mart 1994 tarihleri arasında 5 konjenital tibia pseuodoartrozlu olgu İlizarov yöntemi ile tedavi edilmiştir. 3'ü erkek, 2'si kız toplam beş hastamızın ortalama yaşı 8.3 olup; ortalama 32 ay takip edilen 3 olgu bu, çalışmaya dahil edilmiştir. Paley Kriterlerine göre değerlendirilen hastalanmızdan 2'sinde çok iyi, l'inde ise iyi sonuç alınmıştır. Kaynama ortalama 7.5 ayda gerçekleşmiş uzatma ile de ortalama 3 cm'lik kısalık giderilmiştir. Sonuç olarak bu yöntemle 3 olguluk seride almış oldugumuz sonuçlar memnuniyet verici yöndedir. Vaka sayısının daha da artması ile metodun daha iyi tartışıllabilecağini düşünmekteyiz.
