Posterior Alıcı spinal instrumentation in the surgical treatment of the thoracolumbar and Iumbar spine burst fractures

Abstract

We applied posterior reduction, stabilization and spinal fusion by Alıcı spinal instrumentation to 10 patients with thoracolumbar and Iumbar spine burst fractures between May 1991-May 1994, at the Department of Orthopaedics and Traumatology, Haydarpaşa State Hospital, istanbul. 6 of our patients were male and their average age was 33.2 years old. The mean follow-up time was 21.5 months. The mean Iocal kyphosis angle before and after the operation was 13.9 degrees and -2.8 degrees respectively. The mean Iocal fracture angle before and after the operation was 18.3 degrees and 5.6 degrees respectively. The mean anterior compression percent of vertebra was 39.1% preoperatively, than it became 13.9% postoperatively. The neurologic status of our cases were evaluated according to the classification of Frankel. 5 of our patients who had neuroIogic deficits preoperatively, and other 5 patients who were neurologically intact, all of them were evaluated as FrankeI-E, in the Iast follow-up. We reviewed our cases according to the complications. In 3 patients who had short segment instrumentation: one of 3 patients, one of the upper pedicular screw was in the disc space and one screw was bended at the other patient. In Iong segment instrumentation: Iower pedicular screws were in the disc space in one patienti and four pedicular screws were broken in other two patients. When we compare postoperative and Iast follow-up angles, there was 5.1 degrees Ioss in Iocal kyphosis angle. and was 2. 7 degrees Ioss in Iocal fracture angle.

Torakolomber ve Iomber omurga burst kırıklarının cerrahi tedavisinde posterior Alıcı spinal sistemi

Öz

Sağlık Bakanlığı Haydarpaşa Numune Hastanesi Ortopedi ve Travmatoloji Kliniği'nde, Mayıs 1991-Mayıs 1994 tarihleri arasında, torakolomber veya Iomber bölge omurlarında burst kırığı olan 10 hastaya Alıcı yöntemiyle posterior redüksiyon, stabilizasyon ve füzyon uygulandı. 6'sı erkek olan olgularımızın ameliyat edildiklerindeki yaşları ortalama 33.2 idi. Olgularımız ortalama 21.5 ay takip edildiler. Ameliyat öncesi ortalama 13.9 derece olan Iokal kifoz açısı ameliyat sonrası ortalama -2.8 derece; ameliyat öncesi ortalama 18.3 derece olan Iokal kırık açısı ameliyat sonrası ortalama 5.6 derece; ameliyat öncesi ortalama %39.1 olan omur cismi anterior kompresyon oranı ameliyat sonrası, ortalama % 13.9 olarak saptandı. Olgularımızın nörolojik bulguları Frankel sınıflamasına göre değerlendirildi. İlk başvurularında nörolojik defisitleri olan 5 olgu ile kalan 5 olgu son değerlendirmede Frankel-E olarak değerlendirildi. Olgularımızın enstrumanla ilgili komplikasyonlar yönünden değerlendirildiğinde: kısa segment uygulanan 3 olgudan 2'sinde üst pediküler vidalardan birinin disk aralığına girdiği, bir vidanın eğildiğii; uzun segment uygulanan olgulardan birinde alt pediküler vidalann disk aralığına girdiği, bir diğer olguda iki vidanın pediküler içinde olmadığı, iki olguda ise ikişer pediküler vidanın kırık olduğu belirlendi. Ameliyat sonrası elde edilen değerlerle son kontrol değerleri karşılaştınldığında; Iokal kifoz açısında ortalama 5.1 derece, Iokal kırık açısında ortalama 2.7 derece koreksiyon kaybı olduğu belirlendi.
