The results of surgical treatment of perilunate fracture-dislocations

Abstract

Perifunate fracture-dislocations are rare but serious injuries. Although these can be managed by closed reduction in the acute phase, carpal collaps and postraumatic arthritis are common complications in the Iate phase. Therefore open reduction and internal fixation is mandatory to maentain anatomic reduction and to prevent carpal collapse. The results of surgical treatment of eight patients were reviewed. Four patients were operated on within 1 week after injury. According to the Green and O'Brien scoring system, one excellent and three fair results were obtained in this group Two cases were treated between 15 and 30 th days after injury. The results were poor in both of them. The other two pateints were treated by salvage procedures alter the 30th day one good and one fair results occured. We believe that the result of open reduction and internal fixation is satisfactory in early phase. The salvage procedures are able to fullfill the expectations of patients for restoration of motion, grip strength and pain free function in the Iate phase
