Abstract

Extremity traumas associated with vascular Iesion are probably one of the most problematic cases Objective criteria are necessary for making decision between amputation and Iimb salvage In this study, 31 lower extremites of 29 patients associated with vascular Iesion were reviewed in accordance with Mangled Extremity Severety Score, respectiveif Mangled extremity severity score, there are mainly four criteria; Skeleton-Soft Tissue, Shock, Ischemia and Age. Damar yaralanmalı alt ekstremite travmalarının tedavisi ve ''ciddi hasarlı ekstremite skorunun'' tedavi seçimine etkisi

Abstract

Travma olgularının kabul eden ortopedi kliniklerinde belki de en sorunlu hasta grubunu damar yaralanmalı ekstremite travmaları oluşturmaktadır. Travmalı olguların tedavisinde ekstremite kurtancı girişim veya amputasyona karar vermede objektif kriterlerin varlığı tedavi yönteminin seçimi ve prognozun başarısında olumlu bir etmen olacaktır Bu çalışmada alt ekstremite travması geçirmiş ve tedavisi tamamlanmış damar yaralanmalı 29 olgunun 31 ekstremitesi "Ciddi Hasarlı Ekstremite Skorlaması'' (Helfet) kullanılarak tedavinin başarısı ve tedavi planlamasında skorlamanın yeri retrospektif olarak değerlendirildi. Ciddi hasarlı ekstremite skoru, olgunun ekstremitesinin dört ana başlık altında (İskelet-yumuşak doku, şok, iskemi, yaş) puanlaması ve bunun sonucunda uygulanacak tedavi yönteminin seçilmesine dayanır. Estremitenin puanlanması sonucunda, ekstremite kurtarıcı girişim uygulanan 20 ekstremitede ortalama puan değeri 5.00, amputasyon uygulanan 11 ekstremitede ortalama puan değeri 8.09 olarak bulundu.
