Chiari pelvic osteotomy and femoral shortening in the treatment of congenital hip dislocation

Abstract

The results of 33 CDH cases treated with femoral shortening, open reduction derotation and Chiari pelvic osteotomy are reported. Except four cases, all operations were done in just one surgical prodecure. Patients were aged between four and twelve years and ten cases were bilateral. The results of follow-up for to seven are presented. 11 cases were excellent (33.3%), 10 cases good. (30.1%), 10 cases fair (30.3%) and 2 cases bad (6.1%). The causes which worsen the prognosis were found to be: realization of the osteotomy at a higher level, insufficient displacement, excessive medialisation in one case and deep infection in one case. The results were better in older children and in bilateral cases.
