Osteosynthesis with Grosse-Kempf nail in comminuted and rotationally unstable femoral fractures

Abstract

Fractures of the shaft of the femur are among the most common fractures encountered in traumatology. Since the femur is the largest bone of the body and is a functional part of the lower extremity in load-bearing, these fractures may results in extensive disability or in prolonged morbidity unless treatment is appropriate. Especially the treatment of the comminuted and rotationally unstable fractures of the adult femur have been a dilemma up to date. 31 adult femoral fractures were treated with the Grosse-Kempf inter-locking nail in the Department of Orthopaedics and Traumatology of Medical Faculty of Istanbul University. 22 patients who had adequate clinical and radiographic follow-up are included in our study. The median age of the 19 males and 3 females was 40 years. 17 fractures were due to high energy trauma, 3 were due to gun-shot injuries and 2 were secondary to a fall from a height. The average follow-up time was 26 months. Closed nailing was performed in 14 patients and in 8 patients the fractures site was opened to insert the nail. Radiographic fracture union was achieved at 18 weeks. The result in 15 fractures were classified as excellent; in 5, as good; in 2, as fair and in 1 as poor according to the Thoresen criteria. We concluded that intramedullary locked nailing of the fractures with Grosse-Kempf nail is more superior to the other alternative osteosynthesis materials. This form of treatment has many advantages. Early mobilization of the patient is possible, the risk of infection is low, the incidence and severity of malunion and shortening is prevented and the fracture healing is rapid.
