The treatment of tibial fractures with dynamic axial fixator (Orthofix)

Abstract

In this study, we present the results of surgical treatment of 100 (96 patients) tibial fractures who were treated with Dynamic Axial Fixator (Orthofix) and observed for 34 months (min. 16 months). Ten of the fractures were closed (6 had compartment syndrome), and 90 of the fractures were open (28 Type 1, 32 Type 2, 16 Type 3-A, 7 Type 3-B, 7 Type 3-C). Fourteen of the open fracture cases were treated by minimal osteosynthesis and dynamic loading were not applied to them. Fractures not treated by minimal osteosynthesis, were dynamically loaded in stable group in 4 weeks, in the group 7.5 weeks postoperatively. The period for complete fracture healing of closed fractures was on an average 24 weeks, the average of the open fractures was by Type 1; 23 weeks, by Type 2; 23 weeks, by Type 3-A; 25 weeks, by Type 3-B; 28 weeks and by Type 3-C 25 weeks. Complications were as follows: bending of 8 Schanz screws bone fissure occured with 2 Schanz screw while penetrating the bone, pin tract infection rate was 27% Early union cases of fibula caused a problem in 10 patients. Mechanical insufficiency occured with 2 fixators. 12 tibia resulted with malunion and 7 of them had a shortening (>1 cm). 2 cases resulted with nonunion. We have never seen refracture following the Orthofix removal. According to our results, the successful method for the treatment of tibial fractures, in order to be able to avoid possible complications proper surgical application, experience and strict follow up is necessary.

Tibia kırıklarının tedvisinde dinemik aksiyel fiksatör (Orthofix) uygulamaları

Öz

Bu çalışmada Dinamik Aksiyel Fiksatör (Orthofix) kullanılarak tedavi edilen ve ortalama 34 ay (en kısa 16 ay) süreyle izlenen 96 olgunun 100 tibia kırığı sunulmuştur. Kırıkların 10'u kapalı (6'sı kompartman send.) 90'ı ise açık kırıktı. Açık kırıkların 28'i Tip 1. 32'si Tip 2. 16'sı Tip 3-A, 7'si Tip 3-B ve 7'si Tip 3-C idi. Açık kırık olgularının 14'üne minimal osteosentez uygulandı ve dinamik yüklenme verilmedi (serkilaj teli uygulanan 4 olgu hariç). Minimal osreosentez uygulanmayan 86 kırığın, stabil olanlarına ortalama 4 haftada, stabil olmayanlarına da ortalama 7.5 haftada dinamik yüklenme verildi. Kapalı kırıkların tam kaynama süre ortalaması 24 hafta iken, açık kırıklarda bu süre ortalaması Tip 1 'de 23 hafta. Tip 2'de 23 hafta. Tip 3-A'da 25 hafta. Tip3-B'de 28 hafta ve Tip 3-C'de 25 hafta idi. Komplikasyon olarak: 8 şanz vidasında eğilme olurken, 2 şanz vidası kemiğe gönderilirken kemikte fissür oldu. Çivi yolu enfeksiyonu oranı %27 idi. 10 kırıkta fibulanın erken kaynaması sorun yarattı. 2 Orthofix'de mekanik yetmezlik oluştu. 12 tibiada malunion, 7 bacakta kısalık mevcuttu. 2 olguda nonunion gelişti. Orthofix çıkarıldıktan sonra hiçbir olguda refraktür olmadı. De Bastiani kriterlerine göre tedavisi başarıyla sonuçlanan ve hiç komplikasyon gelişmeyen tibia kırığı sayışı 81 (%81) idi. Sonuç olarak Orthofix tibia kırıklarının tedavisinde başarılı bir yöntem olmakla birlikte oluşabilecek komplikasyonların önlenebilmesi için özel teknik, bilgi, cerrahi deneyim ve yakın hasta izlemi gerekmektedir.
