Polygonal triple osteotomy (Kotz's osteotomy) in the treatment of adult acetabular dysplasia

Abstract

15 hips in 13 patients with acetabular dysplasia were treated by Kotz's polygonal triple osteotomy in the period between 1992-1995. All of the patients were female with an average age of 22.7 years (range 9-33) and the average follow-up period was 28.9 month (range 13-43). The main complaint in preoperative clinical evaluation were buttock pain and Trendelenburg lurch. Preoperative radiographs were obtained in each patient. Average center edge (CE) angle was - 6.2 degrees (-35/+13) and average vertical center angle (VCA) was 0.8 degrees (-45/+29). At final review of these 15 hips, 11 of them have no pain and improvement of Trendelenburg lurch has been achieved in 4 patients but a loss of flexion of 11.3 degrees (0-30). Roentgenographically CE angle of 24.5 degrees (-10/+47), VCA of 27.3 degrees (0-48) in average were observed. Polygonal triple osteotomy can correct acetabular anatomy with its polygonal structure better than periacetabular osteotomies which have difficulties in surgical technique and complication. This technique is a reliable technique in anatomic reconstruction of acetabular dysplasia.
