The effect of external immobilization for lumbosacral motion in sagittal plane

Abstract

In this study, the restrictive effect of flexible lumbosacral corset, Jewett orthesis and molded plastic thoracolumbosacral orthosis (TLSO) for lumbosacral motion on sagital plane of 12 healthy men whose ages ranged between 23 and 41 (average 37.2) was investigated. Lateral roentgenograms were taken in standing position without external immobilization, and in forward bending position without external immobilization, subsequently with flexible lumbosacral corset, Jewett orthesis and TLSO. The angle between L4 and S1 was measured on all roentgenograms. The range of lumbosacral motion without external immobilization was average 33.58±50.88 degrees. With flexible lumbosacral corset, the range of lumbosacral motion was 30.33±6.54 degrees and the restriction of the lumbosacral motion was average 3.25±2.9 degrees; with Jewett orthosis, the range of lumbosacral motion was 24.50±6.63 degrees and the restriction of the lumbosacral motion was average 9.08±4.14 degrees; with TLSO the range of lumbosacral motion was 18.42±5.91 degrees and the restriction of the lumbosacral motion was average 15.17±5.41 degrees. This study showed that, spinal corset orthesis restricted the lumbosacral motion (p>0.01). The TLSO was the most effective orthesis for limitation of intervertebral mobility in the lumbosacral spine followed by Jewett orthosis and flexible lumbosacral corset.

Eksternal immobilizasyonun sagital planda lumbosakral harekete etkisi

Öz

Bu çalışmada yaş ortalaması 37.2 (23-41) olan 12 sağlıklı erkek üzerinde, fleksibi lumbosakral korse, Jewett ortez ve plastik torakolumbosakral ortez (TLSO) ile yapılan eksternal immobilizasyonun, sagital planda lumbosakral hareket üzerindeki kısıtlayıcı etkisini araştırmak amacıyla yapıldı. Çalışmaya katılanların önce ayakta, daha sonra korsesiz, fleksibi lumbosakral korse, Jewett ortez ve TLSO ile gövde fleksiyonda iken lateral grafileri çekilerek L4 ile sakrum arasındaki açılar ölçüldü. Korsesiz lumbosakral hareket genişliği 33.58±5.88 derecede olarak bulundu. Fleksibi lumbosakral korse kullanıldığında lumbosakral hareket genişliği 30.33±6.54 derece, korsenin kısıtlayıcı etkisi 3.25±2.9 derece. Jewett ortez ile hareket genişliği 24.50±6.63 derece, hareket kısıtlayıcı etkisi 9.08±4.14 derece, TLSO ile lumbosakral hareket genişliği 18.42±5.91 derece, hareket kısıtlayıcı etkisi 15.17±5.41 derece bulundu. Bu bulgular eşliğinde, araştırılan üç tip ortezin de lumbosakral hareketi değişen oranlarda kısıtladığı (p<0.01), korselerin hareket kısıtlayıcı etkilerinin ise TLSO, Jewett ortez, fleksibi lumbosakral korse sırasıyla azaldığı gözlendi.
